Symptoms: On January 21, 2021 at the Clinic, a PA was administering vaccinations during the morning for a four-hour shift, 8 AM to noon. At the end of her shift, she handed off her vaccines to another vaccinator. The PA had three doses of vaccine drawn up at that time, in syringes, on her desk. When the nurse inspected the syringes, she found 0.4mL of vaccine had been drawn into those syringes rather than the required 0.3mL. The nurse then alerted the APRN, who escalated the situation to the manager. The manager then contacted Quality. The concern was that the PA had been vaccinating patients all morning with an inappropriate dose of vaccine 0.4mL. After thorough internal investigation, we found that the PA drew up 38 vaccine doses and administered 35 of them. Additionally, based on vial usage for the day and total number of vaccines administered, we have concluded that it is highly unlikely that excess vaccine was administered. This was a near miss event and is being reported out of an abundance of caution.

When the RN inspected the syringes, she found 0.4mL of vaccine had been drawn into those syringes rather than the required 0.3mL. The RN then alerted the lead APRN, who escalated the situation to the operations manager. The operations manager then contacted the Chief Quality Officer. The concern was that the PA had been vaccinating patients all morning with an inappropriate dose of vaccine, 0.4mL. After thorough internal investigation, we found that the PA drew up 38 vaccine doses and administered 35 of them. Symptoms: Patient received that vaccine at 1130 at 1142 patient noted she was having a flush feeling her her face, tingling in her tongue. No noted swelling to lips, tongue, face, no breathing events. Patient was administered Benadryl 50mg IM and was brought for continuous monitoring in our clinic. She noticed relief in flushing within 2 mins of injection and tongue tingling within 5 minutes. It was approximately 30 minutes after Benadryl injection she had improvement with her lip tingling. Patient was monitored in total in clinic for 1 hour prior to be discharged home, no other administration of medications needed. All symptoms resolved prior to discharge by provider.

Lab Data: i didn't call my doctor. I just thought it was a reaction and give it 24 hours.

Symptoms: I had my shot around 10:45 am, was fine my arm hurt, hard when you touched it. In the middle of the night I was freezing cold, headache, heat coming off my body, no temp, everything ache and hurt like I was hit by a truck. I called out of work and slept all day. I only got up to use the bathroom and get a drink. I feel tired today 1/22/21 but chills, hot, aches are all gone. It lasted like 24 hours.

Other Meds: I didn't even take my thyroid medication that day.

Symptoms: Approx 3 -5 minutes after vaccine -redness and hives on both arms, chest, and face. Approximately 10 minutes after vaccine- cough, shortness of breath, tongue itchy, lips swelling. Epi pen in L thigh, 50 mg benedryl IM in R deltoid. Brought to emergency room, 1 liter normal saline, pepcid, solu-medrol, 25mg benadryl IV. D/c to home around noon. 1/22/2020- secondary anaphylactic reaction at 945am. Epi pen to r thigh, 50mg PO benadryl, 20mg prednisone. Drove to hospital ER received 1 liter of normal saline, pepcid, 25mg benadryl IV, 40mg prednisone. D/C at 3pm on prednisone taper.

On 1/6/2021, patient started to experience a rash and difficulty breathing and went to the ER. The ER started her on a new antibiotic. Symptoms: Sore shoulder started after a couple of hours and was moderate by evening. A general unusual feeling started within a couple hours of the shot. By 7pm I started to experience chills and joint fatigue in legs (knees, toes). Chills became severe and a low grade fever of 100.6 developed. Chills lasted about 1.5hrs. Fever broke overnight. By morning all previous issues were resolved and the only issue was a problem with equilibrium and balance (and sore shoulder) It was a general feeling similar to drunkenness. This issue persisted throughout the day and by evening I was experiencing mild vertigo when I closed my eyes. I went to bed feeling somewhat improved relative to all the symptoms but woke having experienced a cold sweat. My shirt was completely soaked. I noted several less significant sweating issues throughout this day with no other side effects. This was the final day with notable symptoms. At the time of this report I have minor shoulder pain as the only symptom that persisted throughout. The equilibrium symptoms were severe enough that I missed a day of work.
