Symptoms: approximately 24 hours after the injection, I had no fever, but chills, sweats and overall since of fatigue. My resting heart rate was at 100 for a few hours. Normally, it's around 76. I noticed on deep breaths, I felt the need to cough (no phlegm, just a dry cough. One or two coughs per attempt). No medical care sought. Those symptoms dissipated and I felt good. Then on January 20 and 21, the chills returned. They lasted about 8-10 hours, then they were gone. I also developed a headache during the 20 and 21 episodes. I checked yes in the below adverse recovery box, but I am uncertain as to what tonight and the rest of the days have in store.

Symptoms: On January 21, 2021 at the Prospect COVID 19 Vaccine Clinic, an orthopedic PA was administering vaccinations during the morning for a four-hour shift, 8 AM to noon. At the end of her shift, she handed off her vaccines to another vaccinator (a RN). The PA had three doses of vaccine drawn up at that time, in syringes, on her desk. When the RN inspected the syringes, she found 0.4mL of vaccine had been drawn into those syringes rather than the required 0.3mL. The RN then alerted the lead APRN, who escalated the situation to the operations manager. The operations manager then contacted the Chief Quality Officer. The concern was that the PA had been vaccinating patients all morning with an inappropriate dose of vaccine, 0.4mL. After thorough internal investigation, we found that the PA drew up 38 vaccine doses and administered 35 of them. Additionally, based on vial usage for the day and total number of vaccines administered, we have concluded that it is highly unlikely that excess vaccine was administered. This was a ?near miss? event and is being reported out of an abundance of caution.

Requested 911 be called. Nurse assisted to lay on the ground with feet up. EPI Pen admin at 11:19am. Reported she felt paralyzed, all symptoms continued, EMS arrived, they administered a 2nd EPI Pen. EMS monitored vitals, O2 remained at 100% but b/p was very low. Transported via ambulance to hospital.

Symptoms: Four hrs. after getting the vaccine. My face started itching, could feel welts under the skin. Swelling of the right side of my face started within a few hours later. Called Drs. , He recommended taking Tylenol. (Second Day) Next day, felt short of breath, weak, applied ice packs to my face. Third day, felt weak. By the fourth day, swelling and itching subsided; but skin on my face felt gritty under the skin. Drove me to the doctor's office the third day. I was short of breath walking from the car to the door to the doctor's office. I was wearing my mask. I saw a PA; but, she didn't tell me much of anything. I don't think she knew what to say.

Symptoms: This reporter was alerted by our vaccinator, that they were notified by one of our teams that they were only able to obtain 1 dose out of a Pfizer vaccine vial. Upon questioning our team we learned that when the preceptor was observing their student, at the dilution step, they reported that liquid was leaking from the connection points of the syringe and needle. They proceeded with the preparation and drew up the vaccine dose, measuring 0.3ml. Patient mentioned here in this report was given their vaccine and observation was completed without remark. It wasn't until the vaccinator was prepping for their next dose from this vial did they learn that the remaining contents of the vial was 0.2ml. The vaccination team then reported this finding to the leads. Patient was later notified via phone (1/18) by reporter of the potential that they received an incorrectly constituted vaccine. Patient reported feeling more pain at time of injection and was markedly tired for a few days after however had no other complaints or concerns at the time of the call and follow up call (1/22).

Symptoms: Patient stated that she was tachycardic the night of her vaccine and then dizzy the next morning. She stated that she was dizzy but that she does suffer from vertigo already. She said these symptoms lasted about a week. On the 1/14/21 she was tachycardic, dizzy, weak and she had a raised red rash distal to the site of injection. She went to urgent care and was diagnosed with cellulitis and prescribed an antibiotic. On 1/17/21 she went back to urgent care since the rash had migrated and appeared patchier than cellulitis. Her doctor said that he did not believe it to be cellulitis at this time and prescribed her a steroid cream.

Symptoms: On January 21, 2021 at the Clinic, a PA was administering vaccinations during the morning for a four-hour shift, 8 AM to noon. At the end of her shift, she handed off her vaccines to another vaccinator. The PA had three doses of vaccine drawn up at that time, in syringes, on her desk. When the nurse inspected the syringes, she found 0.4mL of vaccine had been drawn into those syringes rather than the required 0.3mL. The nurse then alerted the APRN, who escalated the situation to the manager. The manager then contacted Quality. The concern was that the PA had been vaccinating patients all morning with an inappropriate dose of vaccine, 0.4mL. After thorough internal investigation, we found that the PA drew up 38 vaccine doses and administered 35 of them. Additionally, based on vial usage for the day and total number of vaccines administered, we have concluded that it is highly unlikely that excess vaccine was administered. This was a near miss event and is being reported out of an abundance of caution.

Symptoms: On the 1st day, I had a migraine. 2nd day (29th) fever, chills, nausea, vomiting - woke up with it and that lasted about four or five days And I had meds to treat it.. Went to work but had to leave work - acutely ill. When I went home, I had a rash across my abdomen. On the 30th I went to urgent care, I had a rash across my neck when I woke up. Rash kept spreading. They gave me a prescription for steroids and a cocktail of meds to reduce spread of rash. Watched me for 30 min to make sure I wasn't having airway restrictions and they sent me home. Had it for a week and a half. Went to PCP and came up with an action plan to get my next shot.

Symptoms: On January 3, I developed severe pain behind my right scapula. The pain lasted about a week. I subsequently developed right triceps and wrist/finger weakness. I sought care from my orthopedist, physical therapist, and eventually was referred to neurology. Yesterday I received a diagnosis of Parsonage Turner syndrome which may have been caused by the vaccine.
