Symptoms: Patient stated that they experienced "heart flutters" 7 hours post vaccination. Patient stated that they went to their Primary Care MD the next day and was told that they were having "PVCs" based off of the EKG ran by the patients provider. Symptoms: Red, raised bump where injection was administered, the circumference is about 1/4 inch. Directly below the injection site there are approximately 8 distinct scattered red raised dots(bumps) surrounded by cloud-like formation of swollen, also raised dark pinkish skin (surrounding the red bumps), although no distinct borders between the swollen , raised pinkish skin. The size of this swollen area is about 2 inches in diameter, right below the injection site. The said area is very warm to touch, but no pain, however it is very itchy. The bumps do not appear to contain fluid. No break in the skin or bleeding is evident. No nausea, no fainting, no dizziness experienced. However, slight headache experienced. Registered nurse on call recommended washing the area with warm water, applying cold compress and placing steroid cream

Symptoms: Patient with multiple co-morbidities including Gastroesophageal cancer presented to the ER after he spiked a fever of 100.8 F. Admitted with concern for drain displacement and fever on 1/7/21. The patient remained afebrile overnight. Imaging was evaluated by IR and the fluid collection was deemed to minimal to warrant intervention. Surgery evaluated the patient and concluded that no acute intervention was necessary. It is most likely that the patient's fever was a reaction to administration of the COVID-19 vaccine. The patient was deemed stable for discharge on 1/8 and sent home on 5-day course of antibiotics.

The patient was having anormal morning, went to get her first dose of the COVID-19 Moderna vaccine. She got the vaccine, was observed for 20 minutes, left, then about 45 minutes after the dose she started to feel lightheaded. She had some numbness tingling over her face and hands. She felt some tightness in her chest. She felt lightheaded dizzy. She was able to drive back to where she received the vaccine. They contacted paramedics and she was transferred here. The patient denies any swelling anywhere, rash, fevers, chills, difficulty breathing. The patient does have a history of having COVID-19 in September of last yr

Symptoms: Moderna COCID-19 Vaccine Dose 1 was administered evening of 1/11/21. Initially had minor swelling and redness at injection site with minor pain, similar to flu shot, which lasted approximately 48 hours. 48 hours after injection, had chills with no fever for approximately one hour. After that symptom, no concerning symptoms occurred until evening of 1/18/21. Redness, swelling, and itchiness approximately the size of a quarter began. Morning of 1/19/21, size of redness and swelling had increased to size of baseball with itching. Evening of 1/19/21, size had increased to softball with continued itching. Area was raised, hard, and warm. Morning of 1/20/21, area had grown further and was starting to wrap around sides of arm and have ?target? appearance. Saw PCP due to concerns of size growth and itching with no relief from hydrocortisone cream. Recommended use of Zyrtec with hydrocortisone cream and cold compress. Recommended reporting to VAERS.

Symptoms: Pt arrived at the clinic at 1611, pt registered and completed consent for vaccine, provider administered the injection at 1630, Pt. started having throat heaviness and SOB at 1632. Pt. was monitored and she reported face and lip numbness and tingling. She reported hard time swallowing. She was visibly SOB and using accessory muscles bent over. O2 5L applied and Epi was administered. Pt. given epi auto injector 0.3 mg in the right thigh at 1644. Pt. states she feels light headed and nauseated. Pt. throat is patent and LCTA bilateral. Pt. O2 sat monitored during post reaction until EMS arrived. EMS arrived and transported pt. Pt. ambulated to stretcher and was on room air at the time of transfer with EMS. Report given to ER MD at 1730 pm. Pt. was stable at this time.

Symptoms: Within 5 min headache took ibuprofen. 7 hours after vaccine SOB, scratchy throat, cough and voice changes took 125mg Benadryl over a 2.5 hours time frame. Felt ok the next 2 days. Third day after vaccine nauseated but manageable. Fourth day horrible nause that requires multiple medications. Fatigue and chills. Took zofran, reglan, phenergan and ibuprofen. Unable to do anything. Fifth day still nauseated but improved.

Allergies: Reported prior history of allergies to shellfish.

Symptoms: 10 minutes after injection, pt complained of right arm numbness, It began to spread to her throat. She reported throat tightness. , nausea and dizziness. 2 MDs were in the Observation area. They assessed her, gave her 25 mg Benadryl IM and 3 puffs of Albuterol inhaler. Heart rate 110, Resp Rate 12, O2 sat 98% on room air. Pt continued to fell warm and "weird." MDs wanted her to go to hospital but pt refused. Her employer offered to drive her to the ER but she refused. The MDs remained with her, monitoring for another hour. Pt was feeling better and chose to have her husband pick her up.
