Symptoms: For both vaccine doses, does one given 4/12/2021 and does 2 given 5/10/2021 I had a change to my menstrual cycle. Does one on 4/12/2021 caused a period 10 days after administration when the normal cycle should if I stayed on track should have been 20 days later instead. Dose 2 also caused an abnormal/early period 10 days after administration when the normal cycle track should have been 26 days after administration. I do not typically have issues with my menstrual cycle, in fact, they are usually on time within 1 day and have been my entire life. I do not take birth control or have any other issue that I am aware of that would cause this.

Symptoms: Patient stopped by Pharmacy consultation window on 05/19/2021 and asked if arm and shoulder pain could have been associated with the covid vaccine he received. He received his Pfizer covid vaccine on 05/05/2021 in his right arm. He stated that after the vaccine he had a sore arm for a few days and then the bilateral shoulder pain and increased pain in the right arm started a few days after that. He also has arthritis and asked if that could be his arthritis flaring up. He also mentioned he gets cortisone shots in his arms. He wanted to know if he should get his second dose (which would be due in a week) in his left arm instead. I informed him that it's hard to tell and I could not rule out either if it's the vaccine or the arthritis since the pain is in both shoulders and he stated he has a history of arthritis. I encouraged him to check with his physician to rule out arthritis and to determine if it is the vaccine and whether he should receive the second dose. He said he would consult with his physician before his second dose vaccine date. I asked him to let us know what his Physician says and he said he would.

Symptoms: On 5/18/2021, client received her first dose of the Pfizer Covid-19 Vaccine (lot #EW0182, expiration 08/2021) on 5.13.21 and her second dose due on 6.3.21. Using interpreter for this Spanish speaking client who came today with a chief complaint of a very tiny red blister on the left injection site on her deltoid and she said it itches on the injection site and spreading to her chest/around her neck area which started on day two after first dose of Pfizer injection. Lead nurse observed no bumps or blisters on the chest and neck area. Client declined any allergies except allergic to Bacon and declined any chronic illnesses. Client also said that someone told her that the injection site location was not on the right spot. Lead nurse consulted about client's chief complaints with clinical operation lead who told to tell client that the injection site is still within the deltoid area and the red tiny blister is not harmful. Referred client to primary care provider (PCP) to check on her injection site blister and her itchiness on the injection site/around her neck area and to discuss if her PCP recommends her second dose of Pfizer on 6.3.21. Client was provided with all vaccine related information and co-lead nurse educated client about adverse reactions to observe at home and instructed to seek emergency care or medical attention if condition becomes severe after leaving vaccination site. Client verbalized understanding and said will see her PCP before her second Pfizer vaccination. Client left site in no apparent distress.

Both times worse than when ill for 2 weeks from January 7th Diagnosed to Recovery and back to work on January 19th. I think I should have waited to do vaccine until existing anti bodies weaken. No need to be a rocket scientist to understand.

See HPI, physical exam and patient instructions for other pertinent information. Addendum: Pt returned to the office this evening with her husband stating that she felt unwell ~ 20-30 min ago. She says that she felt hot and chilled and felt tightness in her anterior neck. She says that these symptoms have all resolved but they concerned her, so she returned. She still has URI symptoms as noted below in HPI but says that she has not experienced any further SOB. No new symptoms since she was here earlier today. No dizziness, cp, palpitations, jaw/arm pain, diaphoresis. Vitals rechecked here now. EKG done now (declined earlier today) - shows sinus rhythm. Due to the neck tightness, I did advise patient to seek ER care now for further evaluation and management, however, she refuses. I discussed the risks of not going to the ER now, but she still refuses to go. Since she is feeling better, she prefers to go home at this time. She will take a dose of Benadryl when she gets home (incase of any allergic reaction). I again have advised her to seek ER care with any worsening or changing symptoms. On 3/24 he was found out to be having confusion, family called EMS. They tried to debride patient on his feet and he collapsed for about 10 seconds. He did mention that he usually get dizziness/lightheadedness upon standing from bed. Although patient has a history of A. fib but remained rate controlled during hospital stay, we held his antihypertensives including HCTZ and ARBwhile continued his Eliquis. I believe his syncope was likely due to dehydration with decreased oral intake in the setting of acute illness with COVID-19 symptoms. Before discharge is blood pressure was rising so his ARB were restarted while decision regarding HCTZ will be made by primary care physician. In regard to his COVID-19 infection, initially was started on 2 L of supplemental oxygen but his oxygen demand decreased rapidly and most of the time he remained room air, he was not showing any dyspnea, cough, fever or any other generalized symptoms of Covid 19 infection. He did get plasma and remdesivir while inpatient, decision was made to discharge him on 3/26 as patient was symptom-free. He also had received first dose of vaccination. I believe that he would need further remdesivir. We will continue him on steroids for now after completion of dexamethasone for next 8 days he can recent resume his home medication of prednisone.

Symptoms: Pt received shot at 10:49 am. She had warned us of a history to anaphylaxis with penicillin and cephalosporins and we counseled her to be observed for 30 minutes at the pharmacy. Between 10 and 15 minutes after her immunization she had a panicked look on her face and said her throat felt extremely dry and scratchy. We immediately called 911 due to the history of anaphylaxis and they instructed us to not let her have any water to drink. She continued to be alert and breathing/talking through the entire episode and never had any other complaints besides extremely dry mouth, sore throat, and scratchy voice. The paramedics arrived approximately 20-25 minutes after the immunization and observed her in the pharmacy for approximately 5 minutes. They then walked with her to the ambulance where she just sat and talked to them for another 10 minutes and they cleared her to drive home. No medications were administered. A couple hours later I spoke to the patient and she said her throat was still a little sore and dry but nothing else had happened.

Symptoms: I wanted to report an adverse event that I did not include in my vsafe questionnaire due to embarrassment at the time. Around 8PM on the night of my second Pfizer-BioNTech vaccine, I started to notice increased sexual sensitivity in my clitoris just sitting on a chair using my computer. I had started to already feel some side effects, such as fatigue, light muscle pain, and cold. Those symptoms continually got worse and I also ended up having a fever later on starting around 10PM. Symptoms: Client received 2nd COVID vaccine of Moderna (Lot# 021C21A expiration 05/26/21). Client has already completed 30 minutes of observation. At 1800, Client reported to EMT a tightness in her throat and vitals are blood pressure 159/99, heart rate 88, oxygen saturation 97%. At 1801, RN responded to this event. After taking the first set of viral signs, Client states the tightness in her throat has passed and is no longer there. Client reports that her heart was racing when her throat felt tight. Client reports a history of anxiety, takes Xanax as needed, with no known allergies. At 1810, vital signs are blood pressure 169/97, heart rate 88, oxygen saturation 99%. Client reports no chest pain, no tightness in the throat, no headache, no blurred vision, no nausea. RN educated Client about ED precautions and common adverse effects of COVID vaccine. Client was offered supplemental nutrition and fluids. Client accepted fluids, but denied nutrition. At 1818, vital signs are blood pressure 166/90, heart rate 92, oxygen saturation 98%. Blood pressure was taken with an automatic cuff, after this point a manual cuff was used. At 1820, blood pressure is 138/88. Client reports no changes to her symptoms. At 1827, vital signs are blood pressure 122/82, heart rate 97, oxygen saturation 99%. Client reports light headache and reports that she saw white spots when initially entering the observation area but has since subsided. At 1828, left facility with resolved symptoms and with a steady gait. Upon walking out of facility, Client had concerns about adverse effect, PHN reviewed ED precautions, educated about common adverse effects and clarified any questions.

Symptoms: I had a headache in the middle of the night after the shot was given. My arm was sore and then I noticed the next day it was getting red near the injection site. It is now red all around where the injection was given. Similar to what I've see on line. It's a bit swollen and feel bruised. I also want to mention that I have had a ganglion cyst on my right wrist for several years. I noticed today that they cyst has softened and almost disappeared since the second shot. Just wanted to mention this, not sure if it was just a coincidence.
