Symptoms: Patient's spouse alerted nurse that patient was "having trouble breathing". Patient reported feeling dizzy, clammy, and short of breath. Vitals were obtained and provider on call was alerted. Serial vitals taken from 3:05 PM to 3:43PM when Dr. discharged patient home. IM diphenhydramine 25mg administered per physician order at 3:25 PM. Patient reported no trouble breathing following medication administration and dizziness beginning to improve. Symptoms: Syncope times 5 at home starting at 630 am. Called 911. Syncope 2 times at hospital. Hospitalized 2 days until it subsided. Blood pressure kept dropping and heart rate kept spiking. Had chills and aches off and on for 48 hours. orthostatic hypotension. It?s now been 5 days since being released and I?m still dizzy. Feels like I?m off balance/vertigo. Syncope has subsided but dizziness is still present.

Symptoms: suffered a syncopal episode after being administered COVID-19 Pfizer vaccine #1 dose at the conference room downstairs at our clinic. Prescriber came and upon arrival, patient was alert and oriented x 3 laying on the ground. Blood pressure was initially elevated but decreased towards the end of visit. The rest of his vital signs were normal. His lungs were clear to auscultation bilaterally and his heart was regular rate and rhythm. EKG was normal. Patient reports he has had previous syncopal episode when having blood drawn. Patient likely suffered an episode of vasovagal response syncope. Patient was sat upright and sat on a chair for 15 minutes prior to discharge. He has no more syncopal episodes after that and felt back to baseline prior to discharge

Pt also started to c/o mild shortness of breath. ER care on site and evaluated the pt. Repeat BP 162/119. Pt reported he took his Lisinopril 20 mg the night before. Pt signed a refusal for transport to the hospital. Pt was advised to contact his PCP to discuss his symptoms. ED precautions given. Pt verbalized understanding.

Symptoms: Vaccine was administered to patient. Within 10 minutes of administration, the patient had an episode of syncope and fell out of his chair. I immediately went out to help the patient and a technician called for EMS. The patient woke up within 2 minutes and reported feeling dizzy, tired, and numb. While waiting for EMS to arrive, the patient was situated in a sitting position and I continued talking to him to keep him awake. EMS assessed him and took him out to the ambulance to wait for a relative to come. It is unknown if the patient went to the emergency room or not.

Symptoms: 10:05 am Pt received vaccine and was seated in observation area. Pt alert, interacting with staff and other pts. 10:15am Pt appears asleep, rouses with touch and when spoken to. Observation area nurse notified, provided pt with bottle of water and helped her take a drink. Nurse then notified pharmacist who came to pt's side. Pt given 25 mg benadryl caplet. Able to swallow it, needed assistance for holding water bottle. Pt breathing regular, pulse regular and easily palpable. 10:25 Pt increasingly lethargic. Nurse and pharmacist with pt. Lips and tongue slowly swelling, responsiveness decreasing. Pharmacist called 911. 10:30am Pt's daughter administered epipen 30mg, L outer thigh. Swelling of lips and tongue quickly resolving. Pt lethargy increasing. Increasingly slow to open eyes and respond to questions. Pulse and breathing continue regular. EMS arrived on scene approx 10:35-10:40. BP 80/60, NSR, pulses palpable, pupils reactive, able to answer questions but slow to respond and weak voice. Pt transferred to Medical Center. In ER pt given IV solumedrol, IV fluids, CXR, EKG. Awake and oriented, arms shaking, weak, requires assistance to stand, intermittently drowsy. Admitted for observation. 3/3/21 pt preparing for discharge, BP extremely high. Pt given metoprolol, BP increased, tachycardia increased, pt very drowsy. Continued to monitor, given Lisinopril, bp decreasing. Pt feeling much stronger, alert, interacting with staff and daughter. Discharged home.

Symptoms: A headache started in the evening following the injection. At about 2am Friday morning nausea began with passing out when getting out of bed. A fever of 100.2 started about the same time and continued through Saturday when it spiked at 102.6. Extreme fatigue lingered for one week with ringing in the ears beginning on the Tuesday following the injection. My typical resting heart rate is 65 and it was in the 80s at rest and over 115 when walking for 6 days following the injection.

Symptoms: I received my first dose of the Pfizer vaccine at about 4:05 or 4:10 PM today (Wed, Mar 3, 2021). Right away I had the following symptoms briefly: chills and blurred vision. Around 4:20 or so I left in my vehicle. Shortly after, I had a few chills. I needed to use the restroom and pulled into a gas station. After starting to go the bathroom, I quickly began to feel warm and my vision began to blur. The next thing I knew, I opened my eyes laying on the bathroom floor. I then left the restroom and headed to my vehicle where I noticed I had perspiration on my forehead and was really warm. Over the next hour or so I experienced chills frequently. They have since subsided.

Lab Data: I saw my PCP the next day (Tuesday) and was evaluated for possible stroke. There were no signs of muscle weakness, vision, etc. and he/she (2 docs in the room) prescribed an MRI of the brain which is scheduled for next week. He asked when was the last time I had had an a-fib event, and it was Friday night lasting several hours. I agree that some kind of stroke is a likely cause of these symptoms the the timing precisely coinciding with the Day 4 vaccine effects was weird.

Allergies: Rituximab (for lymphoma), get hives

Symptoms: On Monday night, Day 4, I woke at 11 p.m. with chills and shaking, temperature of 99.9 F. Went to the bathroom, urinated, took Tylenol and went back to bed. Woke again at 2:00 a.m, still chilled, and tried to stand up and my legs wouldn't hold me. I managed to lift myself to my feet and clutching furniture get to the bathroom, with complete loss of bladder control on the way. Took more Tyleonol, drank more water, put on clean pajamas but couldn't lift my knees to step into them, had to put them on the floor and pull them up. Went to sleep eventually. When I woke at about 5 a.m., all was completely normal. No fever, legs fine, usual bladder control.

Symptoms: After waiting 15 minutes I was released and felt fine. About 3-5 minutes after leaving the site my tongue became numb and felt s=slightly swollen, throat became extremely dry, pulse rate increased from low 50 BPM to 100 BPM. Drove to a nearby store and stopped, felt a bit disoriented when trying to call my wife. Drank water and waited at a store for approximately 15 minutes until I felt better. This was about 40 minutes after the vaccine was administered. When driving home still some discomfort/numbing of the tongue and throat. Ate at 1:30 and symptoms had stopped.

Symptoms: paramedic called for a 47 female here for first covid vaccination. no prior h/o reaction to other vaccines . pt followed by cardiology for unknown congential heart condition c/b episodes of tachycardia s/p "stress test " a few weeks but unable to complete the stress test. pt pt denies meds for her cardiac condition . approx 9 min after covid vaccine pt c/o substernal "indigestion type" chest tightness a/w sweating and globus sensation of the throat. pt did not have rash or hives or flushing or angioedema or shortness of breath or wheezing or dysphonia or stidor. exam pulse 84 bp 177/112 r 18 afeb pt did not appear in distress heent no angioedema cor rrr without murmur or gallop chest clr p&a. pt refused epinephrine but accepted oxygen. clinic did not have an ekg to assess cardiac status. but emt rhythm strip showed sinus tachycardia. pt transported ED without any medication intervention by emt

Pt also did not mention this to his vaccinator. BP 126/91 HR 95 O2Sat 95%. EMS on site and evaluated the pt. Pt stayed in clinic for over 30 minutes with no progression of symptoms. Pt signed a refusal for transport to the hospital with EMS. ED precautions were given to pt and pt was advised to call his PCP. Pt verbalized understanding.

Lab Data: 02/24/21: Labs normal. COVID test not done. EKG changes noted above. Echo in ER normal. Cardiac enzymes normal. Chest xray showed some nonspecific patchy infiltrates but quality was poor due to underpenetration.

Symptoms: Had COVID vaccine on 02.11.21. States the next morning his anxiety was very high. States he did not know who he was. Still experiencing a lot of anxiety at night. Admits to heart palpitations and hard time breathing. Can't sit still. Reports manic episodes and cycles of depression over the course of hours. Patient states it has gotten a tiny bit better. Tried trazodone which helps temporarily. Denies chest pain. Patient states he has no appetite. Palpitations resolved pretty quickly. Shortness of breath is described as tightness that comes with anxiety feeling. Resolves with shoulder massage. Occurring three times per day. Also admits to dry mouth. Patient admits to needing to sleep with multiple pillows or feeling like he had to sit up and couldn't lay down as shortness of breath would get worse. EKG in office on 2/24/21 showed ST-T changes and given symptoms patient was transferred to emergency room via ambulance. Work up in hospital was unremarkable and patient was diagnosed with pericarditis (unknown etiology) and prescribed NSAIDS from ER and discharged. Admission not required. Patient followed up to us as outpatient on 03.02.21. Patient felt much better and had not take motrin for 1 day. COVID testing obtained at this point (unclear why ER did not send it). Patient referred to outpatient cardiology given risk of cardiovascular disease, stress echo, and event monitor.

Symptoms: I woke up at 6 am on March 3, 2021 with an excruciating pain in my left upper arm that extended to my left elbow. It was a 10 of 10 in severity. It felt like when you hit your elbow crazy bone on something. But it didn't start to subside until after an hour when I put ice packs on it. It was around 3 pm on March 3 when I felt like the pain was bearable. But I had a bad headache so I took Tylenol, which helped. After talking with another pharmacist, at 7:30 pm on March 3, 2021, she said the needles available on March 2, 2021 were longer & thicker than the ones available on March 3, 2021. She said it sounds the administrator may have hit a nerve when he gave the shot. I remember it felt like he had gone much deeper than my first dose.
