Symptoms: at 24 hours, the muscle became inflamed (hot, red, swollen, itchy, painful). Remained so for 5 days, slowly reducing in severity. No noticeable reaction from days 6-8. On day 9 (today), became inflamed again, with welts that look like hives, and the severity of the itching, swelling, heat, and redness started increasing again.

Symptoms: Veteran states two days after his first dose (2/12/2021) he developed a rash on his legs. States the rash is now "up to my crotch, abdomen, and now to my belly button." Also states it is very "itchy" and has been using fluocinonide on it. Veteran was encouraged to be seen in Urgent Care to see if the rash was from something other than the vaccination.

Symptoms: I received the vaccine at approximately 1245 on February 12, 2021. Approximately 12 hours later I got up in the night. I experienced ringing in the ears, sharp intense headache, blurred vision. Experienced confusion. Fainted. Asked my husband to get me Benadryl. Took 50 mg of Benadryl and slept for another 5 to 6 hours. I experienced body aches, chills, headache, heart palpitations, intermittent blurred vision for 4 to 5 days afterwards.

Symptoms: Received dose #2 Moderna vaccine on Feb 3. On Feb 5 woke up with rash over body but not face. Rash more pronounced in bilat armpits. Rash was red and flat and somewhat itchy. Used antiitch cream for symptom relief. On the 9th started getting better and by the 10th it was gone.

Symptoms: One day post-inoculation with the COVID19 vaccine, I experience a spike in body temperature at 99.9 degrees F and chills. Two days following the vaccine, I began to feel nausea with a bit of stomach cramping and chills. I could only eat a milk shake for a meal. Later that evening, I began having diarrhea. By Sunday, I was not eating at all, had a slight temperature of 99.8 degrees F, stomach cramping, chills, and diarrhea. On Sunday night, I began to feel achy all over and still could not eat or look at food. On Monday, February 22nd, I had to stay in bed the whole day as I felt to weak to do anything. By the next day, I began to feel much better and was able to eat solid food again.

Symptoms: Aproximately 2 hours after vaccination experienced chills then approximately 10 hours after vaccination experience face and neck flushing. This flushing proceeded throughout the next day intermittently lasting 30 minutes to 1 hour each time. It is described as feeling like a sunburn. Also experienced soreness and dryness in throat 10 hours after vaccination.

Symptoms: On the 9th day after vaccine (2/25) the injection site started getting a red rash, was itchy, hot and painful to touch. It has gotten progressively worse. Yesterday, 3/1, there was an obvious lump/knot under the skin but has receeded today. About the same time (9th day) started having gastro issues. Stomach pain and vomited on February 27th. Have had stomach pain and diarrhea every day since. I have also had a headache since the onset of the rash. I have not taking anything due to fear of causing more reactions. Only using a cold compress.

Pt discharged to home via W/C to care. Instructed to F/U as noted.

Symptoms: About 20 min after my arm started burning - like someone was holding a match to it. And that stopped. And then I got a shooting pain up from neck to ear; shortness of breath; lips were tingling and mouth was going numb (like a was getting Novocain) and I got a headache and then I got an earache too; I started to feel itchy - all within a short time. I was given Benadryl. It was enough to get my home. I took another Benadryl and I was okay from all my symptoms except the itching (like I was in an ant next) that itching lasted for three days even with taking Benadryl.

Symptoms: Pfizer-BioNTech COVID-19 Vaccine EUA: The evening of and the next day after receiving vaccine patient developed arm discomfort, facial swelling, and extremity rashes that resolved within 24 hours. Patient took acetaminophen for arm pain and diphenhydramine every twelve hours for swelling. During clinic visit to refill anti-hypertensive medications two days after vaccination patient reports symptoms resolved. Vitals within normal ranges except blood pressure elevated. Patient discharged from clinic visit to home stable.

Symptoms: 3/2 I got my first moderna shot yesterday. Last night I started developing chills, a couple degree fever, overall body aches much like the flu and persistent slight nausea. Did not throw up but was uncomfortable for sure. Slept in 2 to 2.5 hour shifts then up to pee and back to bed. Finally this morning I lslept from 10 am to 2 pm. 4 wonderful hours of blissful unconsciousness. My O2 this morning was 98/74 BP 135/81/69. I just took my bp 315 pm and it was running 141 high end. O2 was good at 97 but heartrate was 99 and temp was 99.7 to 99.1.

Did have some swelling and motor loss, NO CP, SOB or sensory loss, no redness, no injury, no fever, chills, ha, depression sore throat, cough, rash, lymphadenopathy, abd pain, GI distress, no trauma or recent illnesses. 2/21: admitted clinic w/rhabdomyolysis: no preceding workouts, increased activity on ship, back sleeper, no alcohol, no trauma,. nsp: skin intact, no erythema. Mild edema to upper arm around triceps. Mild purple discoloration to dorsum of hand. Palp: TTP about triceps. TTP anterior shoulder. TTP biceps. Able to palpate triceps tendon and biceps tendon. NTTP throughout the rest of hand and forearm. Hand is cooler relative to contralateral side. NTTP cervical spine. ROM: Full passive ROM of the shoulder and elbow with pain. No micromotion pain with any joint in the upper extremity. Painless cervical spine ROM. Motor: 3/5 strength to OP/FPL/2nd FDP, EIP/EPL, 1st and 2nd DIO. 3/5 deltoid, biceps, triceps, wrist extensors, wrist flexors. Sens: Decreased sensation throughout entirety of forearm and hand distal to the elbow. Above the elbow, sensation is intact. Vasc: 2+ radial pulse, symmetric to contralateral side. BCR <3 sec. Hand is cooler than contralateral side. Lab: CRP <5, ESR 4, CK 31,924, PT 14, INR 1.1, D-dimer 0.46, AST 610, ALT 186, Cr 0.94, WBC 6.3 Imaging: Radiographs of the chest, shoulder, elbow, wrist, and hand demonstrate no fracture. The shoulder is well located. CTA starting at the heart demonstrate no vascular abnormalities. Assessment: unlikely that he has compartment syndrome or a cervical spine pathology. His exam does not localize to one specific innervation or muscle group. His paresthesias and diffuse 3/5 weakness on exam may be related to parsonage turner syndrome (post viral neuritis) versus edema. While he does have a cool hand and mild/moderate edema on exam, his CTA is normal and his exam is very reassuring that this is not compartment syndrome. He likely has rhabdomyolysis based on upper arm swelling in the setting of CK of 31924. Discharge 2/22/2021: He received a total of 3 liters of IV fluids and was encouraged to drink large amounts of water. Repeat CK levels showed to be down trending as were his AST and ALT enzymes. Plain radiographs of his left arm and shoulder were unremarkable as was CT angiography of his chest. Electrolytes and renal lab work was within normal limits throughout the hospitalization.On the day of discharge, he was tolerating a PO diet, ambulating without difficulty, and hemodynamically stable. LUE w/ minimal swelling, no erythema. TTP over L triceps belly, No TTP over the distal LUE. Somewhat decreased sensation to light touch over distal LUE. 2+ radial pulses b/l. LUE ROM somewhat limited 2/2 pain, but intact. ---- to f/u with HCP in 2 weeks, sooner if worse (medical officer) CK 2/22/21 was down to 14653

Lab Data: His physical exam shows decreased right grip strength, right hip dorsiflexion but all other UE and LE strength normal. Left lid lag which he reports is chronic. His UE and LE DTR's intact including sphicnter tone and anal wink BL. Cerebellum intact to finger to nose BL.

Allergies: atorvastatin - patient reported blurry vision, muscle pain and difficulty with motor control. Cat dander - rash and itching poison oak - rash pollen - itching and runny nose

Symptoms: Pt reported he got the second COVID vaccine at 8:33 am at the Center at an event run by Health Clinic. At 10 am he began to feel unwell describing "disorientation" which upon further explanation seems he is describing difficulty with his proprioception, ie not able to tell where his body is in space, and disequilibrium, ie having to stop and hold on to something while walking. This lasted most of the day and subsided by evening. He has continued to have ongoing shorter episodes 3-4 times per day lasting 5-10 minutes. In addition he has noted increasing weakness in his right thigh, particularly when going from sitting to standing and the through swing while walking. He describes these as "paralytic attacks" that occur once or twice per day and last hours. Both symptoms are similar to those he had post CVA in 2019 and greatly improved with PT and home exercises. In addition he has noted fecal urgency and fecal incontinence in the last couple of weeks, he is unsure if this is directly related to the vaccine.

Cold compress and Benadryl did not change the size or sensitivity of the spot. 3/1- body aches and headache are slightly better, the red rash is slightly less red but the same size.
