Symptoms: I woke up in the morning eight days after receiving Dose #1 to find my upper arm all around the injection site (about a 2" x 4" area) covered in a raised, red, itchy rash--much like poison ivy-type rash. I started applying OTC 1% hydrocortisone cream three times a day and the rash is slowly going away but has not completely cleared up yet. Please note: This is not interfering with my daily activities, but I wanted to report it as a possibly unusual side effect to be added to the information base about the vaccine (assuming a few others may have had it as well).

Symptoms: 2/26/21 @ 1150 Pt in 15min waiting area following administration of moderna 2nd dose. Approx. 5min into wait the pt wife reported something was wrong. Pt was pale, shaking, non responsive and urinated on himself during this event. Pulse was present. Within moments pt became responsive and answering questions. Ambulance arrived, preformed assessment, advised to go to hospital. Pt refused. Pt remained coherent and without complaints.

EKG at Nursing Facility: QT 382 ms, QTC 415 ms EKG device printout summary: "Supraventricular Arrhythmia. Abnormality consistent with anteroseptal infarct"

Symptoms: Excerpt taken from patient's medical record notes. Recreational Therapist was in room with patient who was at baseline mentation at that time, and was getting patient a cup of tea. Rec Therapist observed patient to "all of a sudden slouch down toward left side" and became unresponsive. Therapist called for help, and Registered Nurse (RN) and Certified Nurse Aide (CNA) came into room. Patient was initially non-responsive and profusely sweating and hot to touch. Vitals were taken (see below). The patient then slowly became responsive after approximately 4 minutes and was visibly nauseous and appeared like she was going to vomit, but ultimately did not. Patient was assisted into bed, APRN was called, order for vital signs to be taken every 4 hours for 72 hours, and Electrocardiogram (EKG) to be performed. EKG results were abnormal, indicative of infarct; APRN ordered transport to Emergency Department. Patient returned from Hospital ED at approximately 10:45 PM same day (1/28/21), with transfer paperwork documenting a Hospital diagnosis of "Syncope" without any additional lab results or any new medication or treatment orders in the documentation. Follow up the next day (1/29/21) by RN at the Nursing Facility--RN logged into the Hospital medical Charting system, only to find that there was no record whatsoever of the patient being in the Emergency Department on 1/28/21--no notes, no labs, no summary from a practitioner. Patient recovered and has resumed close-to-baseline activity and functioning.

Allergies: reaction to Humira - central nervous demyelination; leflunomide - hair loss; methotrexate - multiple mouth sores

Symptoms: Slight nausea and about 2:00 am -woke up with an excruciating headache - worse when I was lying down. Sensitivity to lights. Fatigue throughout the day as well. I called clinic at 8:00 am (where I work, too) - spoke with a nurse through triage. They told me to rest and that I should stay on schedule for my second dose (didn't think it was from vaccine). Told me I could take 600 mg Ibuprofen about noon that day and finally started getting relief from headache. I did take another Ibuprofen when I went to bed. On the 13th,I was back to 100%. I don't usually get migraines - it was really weird because I felt worse when I was lying down.

Symptoms: After vaccination with Pfizer vaccine at our clinic, the patient presented her vaccine card to the nurse and it showed that she had previously received the Moderna vaccine. The patient stated that someone at her workplace had scheduled the appointment for her. Our clinic records showed that she had registered to receive a 1st dose and 2nd dose of Pfizer vaccine with us through . The patient was monitored for 15 minutes and did not experience any adverse reactions. She was discharged with instructions that she had completed her Covid vaccine series.

The event occurred at a large vaccine site administered jointly bt. This event is being reported as a vaccine error to assist in data collection and process improvement. When she got into her car she felt chest tightness and her throat was "gummy", also tingling on lips and mild dizziness/weakness. Symptoms: After 10-15min of receiving the second booster I experienced numbness, tingling and muscle weakness in my hands, fingers and arms. This lasted about an hour then subsided. The very next morning I woke up with a bad migraine and a headache that lasted consistently for 10 days. I had intense chills and shaking for three days that started within 24 hours of receiving it. I also experienced muscle aches and pains, fatigue, nausea and tummy aches lasting about 10 days.

Symptoms: On 02/09/2021, the patient received his first dose of the Pfizer COVID-19 vaccine. Two days later, the patient presented to ER at Hospital on 02/11/2021 after noticing right arm weakness, slurred speech and facial droop while in the shower at home. Patient was evaluated and determined to have an ischemic stroke, treated with tPA. Patient transferred to hospital ICU for monitoring on 02/12/2021. Patient transferred to step down care on 02/12/2021 and then was discharged home on 02/13/2021 with improvement in his right sided weakness and the ability to ambulate independantly.
