Symptoms: Woke up the next morning and she felt numbness in her ear. She could hear, but it felt muffled. As the day went on she realized that her ear was ringing. It started as a buzzing and now a high pitched ringing that she still has in her left ear. She also had a nose bleed that occurred twice on her right nostril. She has had nose bleeds in the past, but none recently. She was sitting working and her nose just started bleeding. The vaccine site itself was sore, minor and not unbearable. It's just the humming and buzzing in her ear that will not go away.

APRN Progress Notes APRN (Nurse Practitioner) -- Family Medicine Cosign Needed Expand AllCollapse All COVID VACCINE CLINIC 2/24/2021 Patient: Date: 2/24/2021 Subjective Patient is a 34 y.o. female who was seen at COVID Vaccine Clinic today for her first dose of the COVID 19 vaccination. She was given the Pfizer vaccination in the left deltoid muscle. During her 15 minute waiting period after the injection, the patient began to experience itching and bilateral hand numbness. She denied rash, difficulty breathing, difficulty swallowing, wheezing, throat tightness, stridor, lightheadedness, dizziness, facial swelling and tongue swelling. This provider was notified of patient reaction and she was then assessed in the emergency bay area. Monitored patient for severe reaction symptoms, including but not limited to blood pressure abnormality , hypotension, rapid progression of symptoms, respiratory distress, skin changes and tongue swelling. She developed more itching later in her visit. Including lip, scalp, ears and ears felt warm. ALLERGY REVIEW OF SYSTEMS: Patient complains of itching of skin Patient denies chills, malaise/fatigue, facial swelling, ear pain, sneezing, sore throat, eyes itching, cough, chest tightness, shortness of breath, wheezing, rash, hives, eczema, vomiting, abdominal pain, muscle aches, dizziness and headaches Previous Reactions: none Objective Vitals Vitals: 02/24/21 1030 02/24/21 1050 BP: 124/67 113/69 Pulse: 75 72 SpO2: 100% 99% Physical Exam Constitutional: General: She is not in acute distress. Two doses of 25 mg of benadryl of 15 minutes within each other, pt was able to see improvement. Pt aware of when she is able to take more benadryl if needed. Symptoms: Moderna COVID-19 Vaccine EUA Within a minute of receiving the vaccine, I walked about 40-50 feet and experienced extreme dizziness and shortness of breath, and could not stand straight. I was able to stop my own fall due to a wall being next to me, and I stooped down close to the floor so as to prevent a fall. I then was able to walk about 20 feet to an office where I could sit and the dizziness continued. I noticed I was shaking and heartrate was elevated (that could have been the anxiety caused by the dizziness and being unable to walk straight). I sat for a total of 30 minutes before I walked to my office, which is a couple minute walk. I continued to have dizzy spells (not as severe) while sitting in my office for a few more hours. Also, I'm having a hard time focusing, feel dazed.

Symptoms: Moderate soreness in left arm. I am a physician and I have heard from many friends who are elderly that they had some significant arm pain for 3-4 days after the injection. I wonder if it is because we have had 2 injections in the same place 3 weeks apart. All of them say that the first shot did not hurt. Maybe the 2 shots should be given in different locations to alleviate the pain problem. Just a suggestion. It may be beneficial to study this as well as it would decrease adverse reports.

Symptoms: One week from the time I had the vaccine(2/17/21) my left arm (where the vaccine was done) started to itch and was red and was warm to the touch. That was Wednesday, 2/.24. On Thursday the redness continued around my left arm.It was circling the area where the shot was given. It was still hot and red. On Friday the redness continued around my left arm but now it is in a much larger area.. The redness/warmth to the touch and itchiness area of my arm was getting larger. I went to see the Doctor He did not know if it was an allergic reaction or an infection from the shot. He gave me an antibiotic today which I have not started yet. My arm is very itchy and swollen near the shot area. The skin around the area where the shot was done is hard. I called the Health Dept and I was told to fill this form out. Please advise what I should do next if the area continues to be warm to the touch and the redness/swollenness continues down my arm.

Symptoms: The area by the injection was itching and there was a rash 2 X 4 inches in size. The itching was not severe but it was bothersome. I applied hydrocortisone cream twice a day which helped a lot. I took a Benadryl on day 5. By day 7 the rash was barely noticeable and the itching had decreased. By day 8, all effects were gone.

Lab Data: SARS CoV 2 AB (IgG) Nucleocapsid, QL test (Quest Diagnostic) was negative 3.5 weeks after the first vaccination dose.

Allergies: sulfa

Symptoms: Insidious onset of redness and swelling of the middle and distal phalanges of the 3rd, 4th and 5th fingers of the right hand with progression to swollen fingers with painful finger tips during week 2 of ADR. Initial erythema began about one week after the first dose of Pfizer COVID-19 vaccine. Also redness and swelling of the distal phalanges of the 1st, 2nd and 3rd toes of the left foot with similar onset. Also headache and sore left arm for 2 days after vaccination. Association of "COVID Fingers and Toes" with vaccination was noted about 3 weeks after the 1st dose. Fingers were treated with topical corticosteroid and intense erythema coalesced around the nail beds of 3rd & 4th fingers of the right hand. Also..noted "brain fog" with poor attention and loss of context during weeks 2 and 3 of reaction. Cutaneous manifestations are ongoing.

Symptoms: Pt given 0.5cc COVID #2 injection (Moderna), was waiting in patient area for 30 minutes due to h/o other reactions previously. 20 minutes after injection, pt remarked that she felt hot. Brought back to resp clinic, wasn't having any SOB or CP, didn't appear very flushed. Was shaking. Pulse ox applied, O2 stayed between 98 & 99% RA. HR was initially in high 80's (pt on 50mg metoprolol QD), climbed up to 109. BP (lg cuff, rt arm) was 144/91. Pt became nauseated, vomited phlegm. Did not feel presyncope. Ice applied to back of neck. No redness at injection site. Heart exam: no murmurs or irregular heartbeat; lungs CTA throughout, no stridor or wheezing. Pt remarked that the heart racing, shakiness & heat felt like previous times when she was given racemic epi for reactions. Was monitored for 30 minutes, HR stabilized in low 80's, sensation of heat dissipated. Was allowed to leave 50 minutes after vaccination given, and advised to call for EMT if symptoms came back. She expressed understanding, and was told to NOT drive herself home (family member was with her).

She was given the Pfizer vaccination in the right deltoid muscle. During her 15 minute waiting period after the injection, the patient began to experience itchy, shaky, hot, chest tightness. She denied rash, difficulty breathing, difficulty swallowing, wheezing, throat tightness, hoarseness, stridor, lightheadedness, dizziness, facial swelling, lip swelling and tongue swelling. This provider was notified of patient reaction and she was then assessed in the emergency bay area. Monitored patient for severe reaction symptoms, including but not limited to blood pressure abnormality , chest pain, drooling, hypotension, rapid progression of symptoms, respiratory distress, skin changes and tongue swelling. PMH includes none. She does not take any medications but is on Vyvanse.none. She denies any medial hx. Not on any medications. She did drink a protein shake this am. Symptoms: February 21, 2021 around 4 pm, a woman on the passenger seat of a car, driven by her husband began feeling ? dizzy?. I was approached by our CIO to see her immediately. Another Nurse came to the rescue and we both assessed the patient. She was awake, alert, oriented but anxious and stating she's dizzy. The back of the seat was lowered down for her comfort and blood pressure was taken. It was 172 /100 mmhg. PR = 72 , Temp = 97.5. RR = 18. Two other RNs came to help. The patient was conversant, her husband was upbeat and being funny. Pt started to play with her dog from the backseat reaching for it. The patient was talking but I decided to tell patient to relax since vital signs will be taken again. BP repeated after 6 minutes and its now 155/100, PR= 75, RR=19, TEMP= 97.3, 02 Sat @89 % . Pt?s hands were cold now. O2 Sat was also hard to get a good reading on all of her fingers. Called Dr. Cat Forest and she came to the scene comforting patient while vitsal signs were repeated again after 5 minutes after they were taken Now getting BP= 55/110 mmhg, PR: 80, Temp 97.2 and O2 Sat still at 89% Pt was adviced to go the ER after the Patient?s O2 Sat stayed at 89% and also after finding out that patient haven?t had any physical for two years and that patient doesnt check her blood pressure. Pt and her husband were counselled on the importance of getting regular check up especially at her age of 69 years old. Husband drove patient to the ER per order. Called patient on Monday, 2/23 and Tuesday, 2/24 with no answer. emailed patient and patient answered back and she said that she went to the ER with her husband that day and was discharged and feeling better and wanting to thank the staff for the way she was taken cared of and hoping they will be there for her second dose when she comes back.

Symptoms: Patient reported symptoms of: tachycardia, shakiness, vision changes, tongue numbness and shortness of breath following the first dose of the COVID19 Pfizer-BioNTech Vaccine on 2/5/2021. Patient said that historically has experienced tachycardia and shakiness after getting a flu shot or booster a few years ago. Symptoms reported by patient were enough to suspect potential anaphylaxis, and patient consented pharmacist to administer epinephrine. Patient was then advised by the pharmacist to follow-up with her primary care doctor as soon as possible to report the incident and to discuss the risk versus benefit of the vaccine, whether recieving the second dose would be appropriate for her. Patient returned to assisted living facility for her second dose of the COVID19 Pfizer-BioNTech vaccine on 2/26/2021. Patient said that she discussed the incident with her doctor, was told it would be appropriate to get the second dose despite her reaction and was advised to take "cautious measures". Pharmacist reviewed the patient information and screening questions, discussed the potential side effects and reiterated her risk of experiencing the same reaction with the second dose. Patient stated that she decided on getting the second dose because "her boyfriend is high risk and I'm doing this for him". Pharmacist asked patient if she had any questions/concerns or additional information to add prior to getting her vaccine, and patient denied having any at that time. After administering her second dose, pharmacist instructed patient to stay in the observation area for at least 30 minutes and to ask the staff for water given her experience with the first dose. Patient asked pharmacist how common it was for someone to get a reaction from the vaccine, and then mentioned that her family memeber had to drive her to the emergency room after her first dose becuase her throat was "burning", and was told that her throat was "a little swollen" and recieved a second dose of epinephrine at the emergency room. Patient included this information after the fact her second dose was already administered. Approximately 5 minutes after receving her vaccine, the patient reported similar symptoms that she experienced on 2/5/2021: tachycardia, vision changes, shortness of breath and chest tightness. After monitoring patient with no resolvement of her symptoms, patient was then given a dose of epinephrine by the pharmacist, and the paramedics were called in to assess patient and take her to the emergency room.

Symptoms: Received first covid vaccine 1/17/21 to the right deltoid (Pfizer, lot #EL3248). On 1/27/21 seen by an APNP for right arm pain and decreased mobility. Presented to EHS on 1/29/21 with pain and concern that vaccine was give posterior to the deltoid. Pt did not take pictures or get seen around the time of her vaccine. So, 12 days later, unable to verify exact placement of the vaccine back on 1/17/21. Per the APNP's report on 1/27/21, the pt developed sharp shooting pain into the right trap/scapular area and radiation to lower arm. The pain started within a few seconds after the covid-19 vaccine was given. Feels soreness and tension in the shoulder/scapular area which has not resolved since vaccine. Pt advised continue supportive cares, trial diclofenac gel, use oral NSAIDs cautiously, apply heat. Follow up with EMG (scheduled for 4/14/21). Pt was also given a prescription for cyclobenzaprine. Per documented telephone encounters, as of 2/26/21 pt continues to have significant pain. Note: Pt received her second covid-19 vaccine on 2/13/21 in her left deltoid and there has been no mention of issues with pain in the left arm (Pfizer, lot #EN5318)
