Symptoms: On the 4th day after the initial vaccination shot, I began having chills about 6:00 pm and a temperature of 101.4 about 8:00 pm along with some achiness. My temperature dropped to about 100 and chilling stopped by the time I went to bed about midnight. The nextg morning by 11:00 am my temperature was back to normal. I did experience some tiredness on the 10th but it was resolved by the next day.

Symptoms: Progress Notes Family Medicine Cosign Needed Expand AllCollapse All 2/25/2021 Date: 2/25/2021 Subjective is a 73 y.o. female who was seen at Clinic today for her first dose of the COVID 19 vaccination. She was given the Pfizer vaccination in the left deltoid muscle. During her 15 minute waiting period after the injection, the patient began to experience L arm/hand numbness . She denied rash, difficulty breathing, difficulty swallowing, wheezing, throat tightness, hoarseness, stridor, itching, lightheadedness, dizziness, facial swelling, lip swelling and tongue swelling. This provider was notified of patient reaction and she was then assessed in the emergency bay area. Monitored patient for severe reaction symptoms, including but not limited to blood pressure abnormality , chest pain, rapid progression of symptoms and respiratory distress. PMH: none per pt. Not on any medication. She has not been back to her pcp for "several years". No known hx of htn. She does have a bp cuff at home. ALLERGY REVIEW OF SYSTEMS: Patient denies chills, fever, malaise/fatigue, facial swelling, itching in ears, sore throat, frequent throat clearing, cough, chest tightness, shortness of breath, wheezing, rash, hives, eczema, itching of skin, muscle aches, dizziness and headaches Previous Reactions: none Objective Vitals Vitals: 02/25/21 1118 02/25/21 1136 BP: (!) 172/84 124/82 Pulse: 96 SpO2: 99% Physical Exam Constitutional: General: She is not in acute distress. VS improved after monitoring. Pt monitored for 20 minutes. She feels well. More like anxiety than local reaction. Pt aware of what to watch for. Education on monitoring bp at home and recommended that she follow up with pcp sometime next week to get up to date on her physical and follow up on bp records she will have done at home. Symptoms: This patient presented for her 2nd Moderna covid-19 vaccine. At the time of the administration the nurse administrating the vaccine pushed the syringe plunger after piercing the patient's arm. Some vaccine apparently was injected in the left deltoid while an unknown amount of vaccine leaked between the syringe tip and the needle hub. Due to us realizing that the patient did not receive the full dose and there was no way to objectively quantify the amount of vaccine received the decision was made to repeat the 2nd dose assuring that an adequate dose of vaccine was delivered. This dose was injected today 2/25/21. No adverse reaction was reported.

Symptoms: Within 30-minutes of vaccination, the patient returned to the vaccine clinic to report her face becoming hot, her cheeks being red, and a headache. EMS evaluated patient on-site. Patient was hypertensive (206/96). EMS transported patient to Emergency Department for further evaluation. In ED, patient was still hypertensive, but in no acute distress. Per patient, she was taken off losartan by PCP in September secondary to hyperkalemia. ED physician administered losartan and Tylenol. Patient was discharged with HTN, HA, and vaccine reaction.

Symptoms: upon receiving the vaccine the patients arm immediately turned green and formed small blisters underneath the skin at the injection site. Patient denied previous adverse reactions to any previous vaccines however reported feeling not well after a vaccine for typhoid years ago, patient could not state exact time. 1125 patient bp 211/114, pulse 100, respirations 20 temp 97.8 oxygenation 98%. Patient reported anxiety. Po benadryl offered but patient refused. Patient reassessed bp 172/118. Pulse 69, resp 18, temp 97.8, spot 96%, patient was offered water and taught deep breathing and relaxation techniques. 1210 pm patient stated he was "feeling much better". Site reduced to a small bruise. Pt educated on locL reactions and scheduled for second appt. Consulted with medical director before booking second appointment. Final VS 158/112, pulse 70, resp 18, temp 97.2, oxygenation 98%.

Symptoms: 5 minutes - severe flushing on face/chest and headache, 45 mins - sneezing, 1 hour - itching, chills, 2 hours to 24 hours - continued headache, chills, high fever (102), body aches, nausea, fatigue, stiff joints, tingling lips and tongue, difficulty swallowing, wheezing, MS symptoms, very high sugar levels (450 - out of normal range), began to subside in severity after 24 hours but was out flat for 4 days before feeling better, occassional bouts of chills and itching as well as ongoing need to sleep continued for over a week, huge raised swollen area appeared at injection site - from shoulder to elbow, very hot - used a cold pack to help reduce swelling. Throughout all of this, used large amounts of Benadryl, Advil, Albuterol, drank lots of fluids, used emergency insulin, and had Epi-Pen on hand.

Symptoms: Patient presented t the ED with chest pain, fever and shortness of breath. This was following the 2nd dose of COVID-19 vaccine. She was in her usual state of health prior to vaccine. She had anterior chest pain that started at 1:00am on 2-12-21 along with fever of 100.4, nausea, and HA. Denied loss of taste, smell. No cough or upper respiratory congestion. Pain and local inflammation at the injection site. Also reported right leg cramping and right leg edema. She reported she never had a DVT or PE but has Factor V Leiden. Patient felt that her dyspnea was not related to her asthma, that it felt different. ROS: Positive for fever, SOB, wheezing, nausea, myalgias, HA Physical exam: No acute distress. Tachycardia, No respiratory distress. Breath sounds no stridor, wheezing, or rhonchi. No edema in right lower or left lower leg.

Symptoms: Arm pain, rash around injection site, and periodic numbness which all came on a week after the injection and continue. I take Motrin 2x per day to deal with the discomfort which doesn't completely eliminate it. Also experiencing unusual level of fatigue. These symptoms have been occurring for 7 days now and show no sign of abating. Initial reaction to this vaccine injection was extreme nausea, vomiting, headache and fatigue which began on 02/13/2021 and lasted about 1.5 days. Then I felt fine until the arm symptoms described above began.
