Had slight nausea, shakiness, weakness, little dizzy, headache. Treatment: observation, asking questions, checking vital signs, checked Glucose: Normal. Stayed at Clinic approx. 1 Hour. Considered going to ER but continued to feel better as time passed and went home. BP was at 167/66 when leaving. Later same evening 10:30 p.m. at home: BP at 136/72/66, Next day 2/21/2021 BP at 128/70/63. 2/22/2021 BP 123/66/61. Continue to have headaches.

Patient was diaphoretic, pale with cyanotic oral mucousa/lips. Patient had lost conciousness and regained within 5 to 10 seconds. After coming too, patient was alert and oriented. Monitored patient and she was stable quickly after. Called patient the next day and she reported no adverse effects.

Symptoms: Mild body itching on and off a few days after shot. 9 days after shot woke up with 5 by7 inch red, very itchy oval rash around vaccination site. Lasted 3 days and went away.. However body itching got worse the next several days and has gradually been subsiding but is still present today which is 20 days after shot #1.

Symptoms: Moderna COVID- 19 Vaccine EUA: one day after vaccination patient reported increasing fatigue after flying on an airplane to a high altitude destination. Two days after vaccination patient fell out of bed overnight and awoke confused with a temperature of 104 degrees Fahrenheit. Patient was transferred to another hospital by ambulance, admitted, received antibiotics, but developed acute hypoxemic respiratory failure. Eight days after vaccination patient transferred to intensive care unit at current hospital with acute respiratory distress syndome and bilateral pulmonary infiltrates requiring intubation and mechanical ventillation three days after arrival. Patient remains in the intensive care unit receiving antimicrobials and steroids.

When I looked, the area was pink, raised, with hard lump a few inches below the injection site (which was still bruised). Entire area was pink, hard and warm to the touch. Symptoms: On 2/12/2021, had post-injection symptoms of joint pain, cold sweats and dizziness On 2/19/2021, she noticed injection site was red and raised then became "big, red and swollen" with 3 "bumps" noted. Was seen at Health Center on 2/20/21 and was diagnosed with cellulitis, prescribed Keflex and Claritin. Was seen by this provider on 2/24/21 for follow up and she states that initial red area surrounding the injection site is improved, however, a large red ring is present. Also C/O an additional irregularly shaped red area distal to initial red area surrounding injection site. No infection noted. Instructed to continue Claritin and add Pepcid 20mg daily to help alleviate the histamine reaction occuring. Denies shortness of breath, sore throat, facial/lip/tongue swelling.

Symptoms: normal activity and health after injection at 10:00 am. started feeling ill after a normal nights sleep upon awakening at 5:00 am. felt nausau, drank water, did not feel like myself, did not go to work, Slept fittingly, threw up. spent remainder of day and that night in bed. check for fever several times but never had higner than 98. spent that time with zero energy, no appetite, no energy. 02/24/2021 woke up at 6:00 am feeling like my old self. went to work, returned home, ate moderate lunch, feeling like my old self, may eat normal supper though i still don't have an appetite., i am thinking since i never take any kind of medicene, am seldom sick with any ailment, the covid 19 vaccine was too strong for my system. i am concerned about taking the 2nd dose (due3/15/21). what do you discern? my wife had the vaccine the same day with no side effects. oh, we both had really sore injection sites (could not lift our arms over our heads).

Symptoms: Pain began as a "crick" in the left side of the neck two days after receipt of second covid vaccine. It has now been three weeks and pain has been continuous in the left shoulder and left neck. Pain radiates down the arm and up into the ear, making the ear hurt and making it painful to swallow. The skin is also tender/painful to touch in the same areas, as if nerve damage has occurred. Swelling is visible in the upper back and neck.

Lab Data: No other clear etiologies for acute respiratory failure. CTPA (2/22/21) negative for acute PE, CXR (2/23/21) demonstrated small R pleural effusion and small LLL atelectasis. Patient has been afebrile since admission. Labs were normal: WBC normal, troponin undetectable, procalcitonin undetectable, BNP normal. Rapid COVID negative for acute infection negative x 2. Pulmonology and neurology have been consulted without further understanding for cause of illness. MRI spine pending at the time of this report.

Allergies: No known allergies to food, drug, or otherwise

Symptoms: Patient presented to hospital with increasing SOB, dyspnea on 2/22/21 (vaccine dose on 2/9/21). Patient is still currently admitted to monitor respiratory status and has been diagnosed with unilateral diaphragmatic paralysis.

Other Meds: Atorvastatin 40mg daily, levothyroxine 62.5mcg daily, lorazepam 0.5mg BID prn anxiety (rarely used)

Symptoms: pt states that within a couple of hours after receiving the covid vax she got nauseated, had a headache, stomach pain, dry heaves. Her balance is off and she is having trouble walking. On 2/21/2021 she went to Hospital ER. She was given IV fluids and a shot of Reglan to settle her stomach. Her blood pressure was 250/97. She was discharged w/ instructions to keep drinking fluids, eat when she could and to come back if necessary. Pt has not contacted her PCP at this time. These symptoms have been going on a week now.

Lab Data: They drew blood - ER, January 12, 2021 - normal Next day, January 13, I had tests with blood at my immunologist doctor . All came back. She put me on a regiment on medications - Two Zyrtec in the morning, 1 pepsid in the morning and two Zyrtec at night and 1 pepsid at night - which was what they gave me in ER but she wanted me to make sure I stayed on that continually through second dose and they will ween me off sometime in April

Allergies: I don't know what they are but I do have some - have an epi-pen

Symptoms: For first shot, it was hot right away = I could feel it go down my arm as hot, like fire. As soon as I stood up my scrubs were wet with sweat. They said that I was getting red and flushed. All within a minute. They said my eyelids were starting to swell - upper and lower eye. They had me leave with an ambulance to go to ER. In the ambulance, they started an IV and they gave me Benadryl through IV and they took my blood pressure - it was elevated. I started feeling better. I could breathe. It took 10 minutes to get hospital. The swelling in my face went down. At ER I felt fine. I asked the paramedics ambulance to drive me home but they said that I would just discharged after we arrived. They gave me Pepsid and something else - antihistamine at ER. I had gotten off medication for the immunologist visit which was already scheduled prior to vaccine - but after the vaccine she just got me on higher doses of the medications. February 11th, second dose of vaccine was taken.

Symptoms: I received the vaccine on Saturday 2/6/21 @ 2pm. On Sunday @ 4am I woke with fever 102.5 and chills, overall achy and had flu like symptoms. Maintained fever throughout the day of 100.5-102.5 with Tylenol. Late afternoon I started to feel better. But then at 6:30pm on Sunday evening 2/7/21 , fever spiked to 104.5 chills and shaking uncontrollably. Severe lower abdominal pain and then nausea and vomiting started over the next few hours. Around 9:30pm I had one instance of diarrhea . nausea persisted but no more vomiting. Over the next 48 hours fever of 102 and bloated and very uncomfortable abdominal pain. Mid , left side as well as lower abdomen was sensitive to the touch. On Wednesday 2/10/21 had extremely bloody stools . temp over the following 2 days through Friday 2/12 was low grade 99-100.5 with Tylenol. Cat scan done on 2/11/21 showed Colitis. I have no medical history of Colitis.

Symptoms: 1/22 received 1st dose Moderna. On 1/25 started feeling pain similar to rotator cuff pain in both shoulders. Pain worsened over the next several days and then she also developed pain in her hip joints. 2/1/2021 - 1st appt with doctor, suspected polymyalgia. Prednisone 20mg BID x1 wk prescribed. 2/10/2021 - 2nd appt doctor, pain in both shoulders, possibly related to vaccine, Meloxicam 15 mg daily and labs ordered. 2/11/2021 - Labs drawn 2/15/2021 3rd appt with doctor, Multisystem polymyalgia rheumatica (HCC) . Labs reviewed. Continue Meloxicam 15mg and restart Prednisone 20 mg daily and referred to Rheumatologist. 2/19/2021 1st appt with Rheumatologist, Additional Prednisone 10 mg in the evening, and continue Prednisone 20 in the morning. She has increased her evening dose to 20 mg on her own as 10 mg was not controlling her pain. She plans to contact Rheumatologist about the change in her dose. Repeat labs order to be done in 3 weeks and a f/u appt arranged.

Symptoms: I woke up at 1:30 and my entire body was aching, but like each spot had its own ache -- not a general overall ache. Very hard to describe. I don't mean a little ache here or there. I mean I couldn't turn on either side or flip over. Even the backs of my hands separately ached. I'd never felt like this before. After 15 minutes of trying to figure out what was going on, I got up and took 2 Advils. About 20 minutes later, the aching had subsided.

Symptoms: Fever onset acute with severe fatigue on Friday PM Feb 12. Some lightheadedness when waking up at night on Feb 12 and 13.m needed to sit on the floor to avoid falling. Rapid pulse 80-90 vs normal of 50. Fever to 102 with shaking chills daily for 4 days Feb 13-16. Total days of fever to 100 or more were from Feb 12 to Feb 23. Could not tolerate food from Feb 12 to Feb 22. Lost 7 lbs from 153 to 146. Regained appetite Feb 22. Fever controlled with ibuprophen or Tylenol. After Feb 15.
