Symptoms: Extreme fatigue and dizziness. Mental confusion. Fever. Extreme chills. Within 10 hours of receiving shot. Mild Right arm swelling noted down two fingers wrist pain on right arm because of swelling fingers stiff and painful to move. Could not lift right arm elbow back or up more than 45?. Could not use right arm to lift cup of water to mouth or move pillow. 20 hours after immunization extreme chills unable to get warm. Arm swelling improved hand pain wrist pain improved still cannot lift arm. Unable to go to work. Day two still unable to go to work. Chills improved but extreme muscle aches rash on chest and facial swelling. Faigue.

Symptoms: Red itchy rash on skin about an inch past my injection site, not right at the site, closer to the front of my arm/body. (More anterior?) None It showed up 8 days after my first vaccine. It was about the size of a quarter the first day I discovered it and bigger the second day (today), about 3 inches in diameter.

Symptoms: Very uncomfortable ache in arm for 3 days several days (after initial expected and familiar post-injection soreness that only lasted 24 hours). New ache impacted sleep and daily activities. Feels like what I would imagine a blood clot would feel like but did not notice redness and heat. Discomfort currently focused near inner elbow and underarm area. Symptoms: I developed a 3-4 inch diameter red, swollen, painful, and warm area just below the injection site on my left arm, the day following my second dose of the Covid vaccine. I reported it to today and they have proceeded to treat the reactions as a possible case of cellulitis and they have placed me on dicloxacillin 500mg four times a day for 7 days. I?ve been using an ice pack to cool the area and help with the pain as well as alternating between ibuprofen and Tylenol for pain.

Symptoms: Narrative: Patient with history of seizures was given his 1st dose of Moderna COVID- 19 vaccine toward the end of a stable dialysis treatment. 15 minutes post immunization, patient experienced hypotension and grand mal seizures. Symptoms: syncope, c/o itchy throat Narrative: Patient was seen prior to administration of the Moderna COVID-19 vaccination. He has allergies to bees/egg and a h/o anaphylaxis 2/2 receiving an injection of penicillin in the past. Discussed that prior h/o anaphylaxis to PCN injection was not a contraindication to this vaccination, but is a precaution. Discussed risks of receiving the vaccination vs benefits of the vaccine, which patient states "I already had this discussion with Dr.!" He has multiple comorbidities which place him at a higher risk for more severe illness from COVID-19. Again received the risk vs benefit discussion. Patient stated understanding and desire to proceed with the vaccination. He will be monitored for at least 30 minutes in area where staff can directly observe him. Patient received the COVID vaccination at 13:00. At 45 minutes later, staff alerted that he was c/o palpitations. Due to confusion and weakness, we were unable to transport him to an examination room. Oxygen, water, and vitals machine were brought to him. He presented with SOB, confusion, lightheadedness. Exam demonstrated acute distress, speaking in staccato sentences. HR in 80s, with lungs CTAB and no stridor - despite increased RR to 20s. Vitals with HR in 70s, SpO2 remained 97% or greater on RA. BP was initially 140/72. A code blue was called and Drs. presented to the area. Patient c/o itchy throat and still p/w confusion/combativeness, loss of orientation, and SOB. No visible swelling noticed. At around 13:58, patient syncopized. HR and SpO2 remained 70-80s and >98% on room air respectively. Pulse remained palpable. At that time, an epipen was administered. Patient woke up. During the entire episode, he was able to recognize a staff member with whom he was previously familiar. EMS arrived, vitals remained stable. EMS transported patient to the ER for further monitoring and f/u. Concern for immediate allergic reaction 2/2 vaccination. After event, patient in need of referral to allergy provider for evaluation prior to consideration of 2nd dosage.

Symptoms: Death Narrative: Patient received Covid vaccine on 2/2/21, person reports his legs were more rigid with some sweating the day of the vaccination with leg rigidity that was slowly improving. No other adverse effects reported for following 7 days. Person states he had vomiting episode earlier this week, person states he had no other symptoms before or after the vomiting episodes. On morning of 2/12/21, person reports patient got up ready for breakfast with no issues. She says he asked for chorizo and oatmeal but she laughed and said don't you mean chorizo and eggs. He said yes. They got him into W/C and he was rolling himself into dining room got stuck in hallway. She says he took several breaths then 3 very deep breaths and passed away. She called 911 they took his VS but he has passed. She told them to leave him along no resuscitation.

Symptoms: COVID 19 symptoms and a positive test was confirmed on 1/6, employee noted previous exposure to positive family members Narrative: Employee noted exposure to COVID prior to presenting for 1st dose of vaccine on 1/5/21. On 1/6/21 employee reported the onset of symptoms and was tested and was confirmed COVID positive that day. Positive result was reported to employee health on 1/8/21. Employee Health continued to track employees progress and was informed of the need for hospitalization on 1/14/21. Course of hospitalization noted the need for intubation and significant issue with comorbid condition (rheumatoid arthritis). Employee died on 2/9/2021. Unable to confirm a direct connection to Vaccine vs. COVID infection, but felt it should be reported.

Symptoms: Guillain-Barre Syndrome Narrative: 71 year old male with PMH diabetes mellitus type 2 complicated by peripheral neuropathy and cervical and lumbar stenosis, who presented for AIDP/Guillain barre. Patient was in his usual state of health prior to admission. He had an influenza vaccination late October 2020. About three days later he developed numbness and weakness of the bilateral feet and legs. November 3rd he noticed that this began worsening and raising up more proximally up his legs. he also had bilateral hand weakness and numbness which was worsening as well. He presented to a community hospital 11/5 2020 when he also developed bilateral hand numbness and weakness that has been worsening. He was admitted for AIDP . On 11/8/2020 patient required intubation for acute hypoxic respiratory failure due to aspiration and inability to protect airway. He had piperacillin/tazobactam for aspiration pneumonia completed on 11/25/2020, and then was transitioned to amoxicillin/clav. Hospital course was complicated by AKI secondary to contrast induced nephropathy, which peaked to 4.1 before decreasing back to baseline. He received a total of 5 days of IVIG and also suffered from critical illness myopathy. Also has been treated for dysphagia and was on a honey thick liquids and pureed diet. T2DM managed with Detemir 15 units SC BID. Seen by psychiatry on 11/19/2020 for suicidal ideation and cleared at that time (he was started on sertraline 50 mg PO daily). During his acute inpatient rehabilitation stay he initially suffered from delirium with 1 fall while on the unit. He improved drastically after adjustments to his gabapentin. There was suspicion that withdrawal was contributing to his delirium. QTC noted to be prolonged therefore his quetiapine was stopped. Transitioned back to oral antihyperglycemics early in his stay. Initially he was unable to stand secondary to his AIDP and critical illness myopathy. This has improved with therapy. Patient was also initially constipated, but then began having multiple loose stools after being started on an aggressive bowel regimen. He was tested for C diff, which returned negative. Bowel regimen has been titrated down to senna PO QHS. He initially suffered from areflexic bladder with elevated PVRS and no sensation to void. He was trained on intermittent catheterization and sterile cath technique. He eventually developed sensation of needing to void and began having continent voids. Patient had complaints of frequent urination from 4-8 pm. PVR bladder scan followed by a straight cath PVR were obtained, and returned low. It was hypothesized that due to fluid mobilization at the end of the day when patient elevates his BLE triggered his need for frequent urination. He was supplied with a urinal to use in the evenings to minimize the risk of falling. It was discussed that patient may benefit from a urodynamic study in 6 months from discharge. On 12/17/2020 he had acute onset of chills, feeling of recurrent weakness, and malaise. URine found to have bacteria. This was the second UTI he has had since onset of AIDP. CT ordered for R/O Bladder stone, which returned without kidney stones nor bladder stones. However an incidental hyperechoic mass on the kidney was seen bilaterally, with recommendations for MRI renal protocol. Initially patient was placed on trimethoprim/sulfamethoxazole, but due to urine culture resulting with E. coli, he was transitioned to cephalexin (finished a 14 day course) and started on methanamine for prophylaxis. Patient was interested in work up of his BUE tremors while he continued to progress in therapy. He was evaluated by neurologist. Labwork work up was unremarkable and symptoms deemed likely secondary to benign familial tremors. Since patient's quality of life was unaffected and he did not have social embarrassment from his tremors, he declined pursuing medication treatment. Plans are for him to follow up with provider in 6 months from discharge. Patient continued to do well throughout his stay. After being seen and examined by attending physician on 1/22/2021, he was deemed stable for discharge home.

Symptoms: 1/26/2021 Swollen hand/redness/painful . 1/27 Urgent care possible infection. Doxycycline HYC 100mg 1 capsule x2 for 10 days. Naproxen DR/EC 375 mg 1 tablet x2 for 7 days. On 2/5/21 hives all over and hitching ( see pictures). Allergist prescribed Prednisone 5 mg 8 day for 3 days ( 2 Benadryl twice a day for 1 day) as of today 2/12/21 down to 2 prednisone a day until 2/14 21 and Zyrtec 10 mg at night. Symptoms: I woke up on the morning of February 1st with a rash, itching, and swelling at the injection site. It was in the shape of the oval that was at the site for several days after the injection, but the mark had cleared from my skin prior to this reaction. I used Benadryl and hydrocortisone cream for three days to soothe the itching. The swelling cleared after approximately five days and the rash after a little over a week. I still have a slightly darkened oval shaped marking at the injection site but there is no longer any swelling or irritation.
