Symptoms: day 1 (Feb01) rt arm achy and sore to lift over head and first night could not sleep on rt shoulder. Slowly got better over a few days. Day 9(Feb 09) rt upper arm itchy and red blotch area about 2x2 inches. I was able to sleep on rt side that night. Day 10 (Feb 10) red blotch area bigger about 4x4 inches and a little less itchy. I started to get concerned and google side effects and came across (covid arm) that looks like my arm.

Symptoms: Copied from Clinic Progress Note dated 2/5/2021: "Last seen in our clinic 1/28/21 with progressive joint stiffness, arthralgia and "burning of hands" starting a few hours after she received the COVID vaccine on 1/14/21. Unclear etiology. Labs including CCP, rheumatoid factor, SLE panel, SSE Ro and ENA all returned unremarkable. Rx protonix for increased ibuprofen use, Celebrex and prednisone (short burst) and advised to follow up in 5 days. Pt was seen 2/2/21 in the ED for myalgias, fatigue, progressive paresthesias in hands intermittently x 2 weeks. EDP discussed with neuro who recommended CT/CTA, MRI, LP which all returned unremarkable. CBC, CMP, ESR, CRP, TSH, CK, and multiple sclerosis labs all returned unremarkable. No focal deficits on exam but did have positive Phalen's and Tinel's. Discharged home with R wrist splint and Rx Neurontin BID for neuropathy. Has scheduled follow up with PCP on 2/10/21, but today is requesting a excuse from work note. She works as a CNA. Has not worked in 3 weeks. Right hand goes numb and she drops things. Requesting light duty work note for at least next week. Gabapentin 300 bid started from ER. She reports much improvement in all over body pain, but does make her feel she has a flat affect when she takes it during the day."

Symptoms: Covid vaccine on 1/30. On 2/7 in the evening: the injection site had a small 1-inch red spot. Then small red itchy bumps appeared on elbow of injection arm, on both thighs, behind knees, and scattered on hands, feet, chest, face, and back. Worst on thighs/legs. On the morning of 2/8, Urgent care prescribed prednisone and benadryl. Became increasingly itchy day 9-10 and started to bruise at the center of each red spot. Redness spread. Today is 2/10 and the redness/hives/itchiness appears to be subsiding. Followed up with primary care doctor today (2/10) and he confirmed treatment seems to be working. Doctors do not think it is related to the vaccine, but I have seen others online talking about full body rash/hives even a week to ten days after the shot, so I wanted to report just in case. I also had a rapid COVID test done last night (2/9) and it was negative.

Symptoms: Pt is a 33 y/o male who presents to the ED with c/o of left le swelling progressing over the last 4 days. Pt has no other c/o. Denies fever, cough, cp, abd pain, n/v/d. Pt has had his first COVID vaccine 1/30. Patient was noted to be tachycardic to the 120's in the ER, after 1L of normal saline still tachycardic to the 110's. Lower extremity doppler was negative for DVT, however PE protocol CTA of the chest was positive for a PE, report follows. Patient was given lovenox and flown to Medical Center for higher level of care.

Change in mental status, acute ASSESSMENT/PLAN/DECISION MAKING: Spoke with radiologist informed me that patient had a deep ischemic or infarct change in his corona radiata area which is common in patients with covid 19 infection or vaccination, which patient had a day and a half ago after lengthy discussion with wife, she informed me that patient would not want CPR and he had a stroke in the past and does not want to go to another city as he is 91 years old and therefore not a candidate for thrombolysis and also this was a wakeup stroke and so we do not know his exact time of onset, and she states that they simply want to be treated here for comfort care.

Symptoms: On the evening of January 30 I began having chills and shaking. I then went to bed and had difficulty moving in the bed. I went to sleep. When I awoke on the morning of January 31, I was unable to sit up in bed and had been incontinent during the night. My wife called the paramedics to help get me out of bed and to change my clothing. At this time I refused to go to the hospital. The paramedics wheeled me to my lounge chair in the living room. About 1:00 p.m. in the afternoon I asked my wife to help me get out of my chair so that I could take a shower. I was not able to get out of the chair, stand or walk. My wife again called the paramedics and I was transferred . I was in the emergency room until 10:00 p.m. that evening. I was given some fluids IV. The ER staff had me walk to be sure that I was mobile before discharging me. They could not say for certain if this was a reaction to the Moderna vaccination, but thought it could be. Within a few days I spoke with my primary care physician, who concluded that I did in fact have a reaction to the vaccine. She has recommended that I not get the second shot at this time.

Symptoms: Beginning 10PM (11 hours post vaccine) I experienced severe body pains and was unable to sleep. Called out sick from work. Took 800 mg OTC Ibruprophin every four hours, which only reduced pain for an hour. Temperature went up to 102.1 morning of 02-09-21. Severe body aches and fever continued until 2:00am on 02-10-21, when the fever broke the body pain subsided as well. Overslept morning of 02-10-21, but was able to report to work an hour late. No side effects at this time, except for dull pain in site of injection.

Symptoms: On February 4, 2021, patient woke up at 3 a:m, left eye kept opening and closing, vision was blurry, and left side of face numb by lips and jaw. She took 2 Benadryl and took a nap Thursday afternoon. She went to the ED later and was told it may be Bells' Palsy but was referred to PCP. Symptoms: Feb 4, 2021 - 7:35 pm - I received the Moderna Covid vaccine in my left arm. 9:00 pm - I started having bad arm pain Feb 5, 2021 - At about 1 am I had to leave bed to sit in a chair because it hurt too bad to lie down with my arm pain and I could not sleep. I continued to have bad arm pain all day. A cold wash cloth helped a little. I tried rubbing it, but that did not seem to help. It hurt so bad that I could not lift my arm over my head. Feb 6, 2021 - At about 6 am, I had a bad headache which continued all morning. At about 12:30 pm it became severe. I did not want to take any pain relievers in case they would interfere with antibody development. However, my head pain became unbearable so I took two Extra strength Tylenol at 1:20 pm. (This was about 30 hours after COVID shot. I was even concerned about having a stroke or other health problem due to extreme head pain.). By 2:00 pm I was feeling much better. The headache was gone by 6:00 pm without taking any more Tylenol. My arm pain was still there, but was a little better. Feb 7, 2021 - My arm pain was still there, but was a little better. It still hurt to raise my arm. Feb 8, 2021 - I only had minor arm pain. Feb 9, 2021 - Basically all better with just very minor arm soreness.

Symptoms: Patient called in to the nursing supervisor at approx 02:40. She was not on the schedule but was stating that she was having a reaction to the Covid vaccination she recieved the evening before. It was noted that while she was talking with the supervisors her speech was thick and she stated that her tongue felt swollen and larger than normal. She also stated that her significant other told her she sounded different too. She stated she had redness from her earlobe to her wrist on her R side, which is where she got the vaccination. She got it at appx 18:30 she had said, and when she got it during the observation period she was noted to have a very red and hot arm, but overall she felt fine. She also stated that she has had a covid infection recently. She stated she took 1 childrens dose of benadryl but was worried because she is breastfeeding. We advised her to call 911. *** 1000am- #2 vaccine given on 02/9/21 @ 610pm. felt ok for the first 15 minutes during the waiting period; the site was examined by the Medical assistant & was very hot to touch & red about the size of a baseball. She was dismissed from the vaccine clinic. She went home and around 7:30 pm felt swelling in her tongue & mouth. She disregarded this symptom and went to bed, she awoke @ 2:30 am and felt her tongue was more swollen & then called the nursing supervisor. Patient took Benadryl 50 mg at 0230am. NO response from the Benadryl. Went to the ER @ 0400 called 911, B/P was 155/88 & O2 sat 89%. She opted to go to the ER by private vehicle. Went to ER. She was give IV fluids, Benadryl & monitoring. Discharged from ER this am. Hx of allergy of bee stings. Stated this reaction was the same as a bee sting reaction. Discharged from the ER to take tylenol & Benadryl & monitor. During this call she stated that her tongue is still a little swollen, speech is clear, Denied SOB. Temp 99.8 during this call. Lymph nodes in her in neck are swollen- she said there is 1 area near her right collar bone the size of a golf ball. Currently nauseated, denied chest pain. She does not qualify for testing since she had Covid on 01/8/21. After asking the patient if she's experiencing any other symptoms, it was determined that this is not a severe allergic reaction needing a more intensive intervention.
