Symptoms: I got my 2nd shot around 10am. My arm started to feel sore within 1-2 hours. At 7pm I started to feel pain in upper left back area with breathing. At 9pm I was in bed and started to have chest tightness and pain with breathing. Both arms and hands started to go numb and tingling. I got to ER around midnight and all testing was negative. I was hooked up to IV fluids which seemed to calm the chest pain and numbness. I got home around 3am and went to sleep. By 7am I was having sore muscles and body aches. I was fatigued and having chills. I was also running a low grade fever. I continue on like this for 24 hours. The next 24 hours I was fatigued. By day 3 I felt like I was getting back to normal.

The patient had a COVID shot this morning, and shoulder was hurting so bad, she was going to take a Tylenol 8 hour, wanted to know if that was ok, and what about heat or ice. The Tylenol said for muscle aches, but she didn't know if she should take it, it had been hours since the vaccine, hurting a little bit, certain ways move it, was going to put heat, thought maybe ice. It was just a little sore, nothing she can stand, she didn't know whether to take a Tylenol for muscles, then heat or ice. The patient's medical history included vertigo (she hasn't had vertigo in the last ten years, she did not remember when the first time had it, it was years ago), congestive heart failure (CHF, diagnosed a little over a year ago), diabetes (diagnosed about 10-15 years ago, her A1C is always good, she has good control over that), and she got a pneumonia thing but not recently in the last 6-8 weeks. No further details provided about unspecified pneumonia thing. There was no concomitant medications, also she has no other recent vaccinations or started no new medications. Patient reported that she had been having challenges with her blood pressure going up after receiving first Pfizer COVID vaccination. Her appointment of 1st dose was at 13:45, she probably got it close to 14:00, she was out of there by 14:15-14:30. Patient reported that ever since she had the shot, her blood pressure continued to go up, so she would like to ask if she should get a second dose. She takes her blood pressure every morning since she was diagnosed with CHF and they advised her to take that every day and weigh herself to make sure she is not getting fluid build up. Patient said that on 16Jan2021 when her blood pressure was 169/91 she woke up very dizzy and thought she was coming down with a vertigo attack, which she usually has had in the past that when she had attacks her blood pressure doesn't go up, so she realized her pressure was causing her to feel dizzy. She took it later on in the day and it was 157/84. She looked back and every day it had gone up, and it fluctuated. She says on the time of reporting (on the 25th), it was 160/90. She is scheduled/due to get her second dose of the vaccine on 02Feb2021, and she could contact her doctor for her question. The patient medical history and concomitant medications were not reported. The patient was tested positive for covid 19 on 19Jan2021 so she has been quarantining. In Jan2021 she had a 100.5 fever for one day, nasal congestion, aches and pains, stomach, acid reflux. The patient underwent lab tests and procedures which included body temperature: 100.5 fever, Sars-cov-2 test: positive on 19Jan2021. On 25Jan2021 was the best she has felt, where she was getting my energy back. As of 25Jan2021, her symptoms have been diminishing. She typically got sick when she got the flu vaccine or any vaccine in general for immunization, she got deathly sick. She always got the flu after getting the flu vaccine. She did not get the flu shot this year and she did not get the flu. She did not know the name of the flu vaccine or any vaccine that she was referring to and she did not have lot number to provide. Other medical history was reported as none. There were no concomitant medications. Relevant Tests were reported as none. The patient experienced lost sense of smell on 23Jan2021 with outcome of not recovered, feeling really tired on 24Jan2021 with outcome of not recovered, chills and sweats on 24Jan2021 with outcome of recovered on 25Jan2021, sore at injection site on 24Jan2021 with outcome of not recovered, body aches and Shortness of breath on an unspecified date with outcome unknown. Details were as follows: the patient had a list of the expected side effects and she did not see loss of sense of smell on the list. Reported she completely lost her sense of smell, she cannot smell anything at all. She was asking if she should be concerned. On Friday prior to getting the vaccine she could smell everything. She thought the loss of smell started Sunday, reported it may have started Saturday (23Jan2021). She noticed when she put her body spray on Sunday and she was washing her hands that she could not smell. She can taste fine. She can handle the body aches and all the other stuff. She clarifies she was experiencing body aches, she was feeling really tired, fatigued. She was feeling better. She could not call earlier because she was so tired. She had the chills and sweats that started Sunday evening and recovered today (25Jan2021). Reported her sheets were drenched with sweat when she woke up. She did not have a fever. The patient woke up early at 6:00am on 24Jan2021 with really bad chills, fever and joint pain. Her temperature was 101.6 after taking Tylenol. Her arm felt like there was a small "know" in it. Some soreness, but feeling better. The patient had a really bad day yesterday on 24Jan2021, she experienced really hard chills about 5 or 6 episodes of chills. Her temperature got up to 101.6 and that was with Tylenol. She felt better today and her temperature was back to normal on 25Jan2021. She had soreness, achiness, she was still sore in regards to her muscle. She got over covid. She was diagnosed with covid on 23Dec2020, about 30 days ago. She got the first dose of the vaccine on 23Jan2021 and she was scheduled then since she was told to wait about 30 days after being diagnosed with COVID. She heard the second shot might be more intense and asks if that was right. She called the doctor. When querying lot number, she put her glasses back on to read the stamp on the card. She had to get under the lamp since it was kind of blurry. She could not read the lot number on the card and had to get a magnifying glass and then was able to provide lot. The first episode of chills happened during the night and woke her up about 6 am on 24Jan2021, when she got up she was really stiff and she could not walk real good. She had to hold on to something when she got up to go to the bathroom. The chills that were so bad were yesterday from 11 till about 4 or 5. Around 6 or 7 pm yesterday she had lighter chills. Concomitant medications included amlodipine started taking it years ago and hydralazine both for high blood pressure, bupropion, iron started a year and a half before (in 2019) for iron was low, all ongoing; donepezil started taking it in 2011 or 2012 as memory medication, apixaban (ELIQUIS) started taking two years before (in 2019) for blood clots, isosorbide started it so many years for heart, quetiapine from 2011 for having problems with memory. The patient previously took another medicine for blood clots. The patient went to hospital for the following side effects: dizzy and nausea on 23Jan2021, sharp pain in stomach in Jan2021, high blood pressure on 24Jan2021. Reporter would like to know if this side effects were reported or what should she do for her mom. Patient had just not being well in Jan2021 after received bnt162b2. Reporter had to take her mother to the ER 24Jan2021, and right now patient was dizzy, had nausea, and reporter took her to the ER yesterday and they couldn't find anything, and reporter was trying to find: was this due to the vaccine. Patient said she didn't feel well, since Saturday (23Jan2021). Patient now had a lot of stomach pain, so reporter took patient to the ER on 24Jan2021, and did a lot of tests, and her blood pressure was sky high, reporter wondering did the vaccine affect blood pressure. Patient was just not herself, and reporter knew the patient, patient had been doing pretty good, but was not herself. She explained the first instance she experienced rigors was after a lymphoma treatment. She believed the rigors lasted "12 hours" but was not certain because she "fell asleep and got warmed". The patient felt great all day on 21Jan2021 after receiving her first COVID-19 Vaccine dose. She said almost exactly 12 hours later, she started to have rigors. She was shaking so hard that her hands and body were jerking. Her muscles and joints were hurting, her equilibrium was off, she ached all over, she had pain, felt tired, and she had chills. The rigors started first. She said she felt really cold at the same time, so she went to bed and covered up, trying to get warmed. She fell asleep for awhile, and then woke up and went to the bathroom. When she went to get out of bed to go to the bathroom, she stood up, and every bone and muscle in her body hurt. She said she was aching all over, and her equilibrium was off. She didn't feel like getting up, and finally went to sleep to get the rigors to stop. She felt cold all the time now, but the rest of her symptoms had improved. She said it seemed like the muscle in her right leg was still pretty sore. She said she recovered from the rigors completely. She had rigors once before when she had an infusion. She clarified the infusion was Rituxan. She said she had a reaction to her first treatment of Rituxan. She said she had rigors when the Rituxan infusion was first started. She was given a couple things (but she didn't know what those things were), and she stopped shaking. She said the infusion staff waited a few minutes and tried to restart the Rituxan infusion, but she would have rigors again. She said the infusion staff tried slowing down the Rituxan infusion rate, as well. She said the Rituxan infusion normally takes 2 hours, but after 8 hours, she had only got about 20% of the Rituxan infusion, before the Rituxan infusion was stopped. She stated she had 3 subsequent Rituxan infusions at later dates, and did not have any problems with those infusions. Her first dose of the COVID-19 Vaccine was administered high up in the muscle of her left arm, close to the shoulder joint. She said the COVID- 19 Vaccine injection didn't hurt, and her left arm never got sore. She said on 30Dec2020 she had lab work done for her oncologist. She went about every other month for blood work to make sure she was still in remission. She saw her oncologist on 06Jan2021, and her oncologist said her blood work looked pretty good and was better. She asked her oncologist about the COVID-19 Vaccine, and her oncologist said she definitely wanted her to get the COVID-19 Vaccine. Her next appointment with her oncologist was in Mar2021. She said she had been seeing her oncologist every month, but her oncologist this time, wanted to try seeing her after 2 months. The events did not require a visit to emergency room or physician office. The outcome of events rigors, shaking was recovered in Jan2021. The outcome of events muscle and joint aches/her muscles hurt, equilibrium was off, aching all over, tired, bone and muscle in her body hurt, hands and body were jerking was recovering. Other medications in two weeks included low dose naltrexone and salbutamol sulfate (ALBUTEROL [SALBUTAMOL SULFATE]) and Herbals. No other vaccine in four weeks. Patient previously took erythromycin and epinephrine and had allergy to them. Within 5 minutes (21Jan2021 12:15 AM), her system felt ramped up. Her heart rate went up, not extremely high but and EMT said it felt "racy". She was told that her voice sounded worked up. she had slightly shallow breathing. It lasted about 30 minutes then began to subside. The next day she felt it somewhat. she was totally normal after 12 hours. She had a sensitivity to epinephrine and the Pfizer dose reaction reminded her of a milder version of the epinephrine reaction. No Covid prior vaccination, no covid tested post vaccination.
