Symptoms: A large circular, red, itchy lesion appeared at the vaccination site. It was swollen and hard to the touch. It grew overnight and into the next day reaching a diameter about 4 inches. It healed and disappeared after 3 days. Itchy hives on the buttocks, back, and stomach occurred at the same time, appearing each night for 6 days. Benadryl was used on nights 3 and 4, and hives persisted.

Symptoms: 2nd dose- from 700pm on 2/1/2021 to 4pm on 2/2/2021- I had severe muscle and bone aches in my hips/ legs/ feet, fever up to 101.4, chills, severe headache and neck pain, eye pain, fatigue. i had on and off again nausea. I did have a sore arm for the next 4 days. I took 400- 800mg Ibuprofen around the clock from 7pm on 2/1/ 2021 to 1230 on 2/21/2021 and switched to 1000mg of tylenol on 2/2/2021 T 9PM. 1st dose- I had sore arm for about 4 days and 4 headaches in one week. I was also tired on and off for the next five days.

Symptoms: Patient received injection and then started feeling heart racing like palpitation, dry mouth, flushed feelings. Patient stated she did not eat breakfast and has not been drinking water. Patient also states she has a history of panic attacks, anxiety, IBS, Cevo. Patient did have c/o a slight headache however, felt it was due to lack of water intact today. Patient sat for remaining 30 minute observation with nurse and provider on duty and was given water. At the end of the 30 minutes patient had decreased symptoms and stated her husband drove her. Provider gave ok for patient to take a prescribed Lorazepam when arriving home today. Patient discharged home after 30 minute observation period was complete with husband.

Symptoms: Cough, shortness of breath, headache, dry mouth, fatigue, body aches, and runny nose, all starting 2/4/21. No known COVID-19 exposures. Patient presented to ED on 2/8/21 stating she was using her albuterol inhaler frequently with no relief and felt like she was having difficulty breathing. SpO2 was 99% during ED visit on room air. Patient had a negative rapid COVID-19 test, however chest CT with contrast revealed pulmonary infiltrates consistent with COVID-19 pneumonia. Patient will have PCR testing performed, results pending. Patient is being discharged to home with azithromycin, and prednisone. She has been instructed to continue self-isolation until the PCR test results.

Symptoms: Experienced pruritis immediately after injection. Pt had pruritis with first vaccine 1/15/21 however, did not report symptoms and took Benadryl 50mg po and symptoms resolved on their own. Pt thoroughly examined, no swelling noted to tongue, lips, uvula midline, vss, no urticaria present, no nausea. Pt. Given 25mg po of Benadryl and observed for 30 mins: after and remains without s/s of anaphylaxis. Pt. Reports improvement of itching.

Symptoms: Patient at site for 1st dose of COVID Vaccine. Patient approximately 2 minutes after receiving vaccine at 1028 started feeling tongue swelling with throat itching. Patient taken to first aid room where she was assessed by provider and ems on duty. Patients skin was red and flushed and with slight swelling of the tongue noted. Patient still A7 O x4 with respirations even and unlabored. Patient had O2Sat of 96% on RA. Patient states she has an allergy to all Bovine proteins and had a mini-stroke approximately 4-5 years ago. Patient is a healthcare worker and is currently at home with newborn son. Provider ordered Benadryl 25mg PO and pt received 1st dose at 1045. Then provider ordered a second dose of Benadryl 25mg after patient stated her Jaw felt "Wonky" and patient received at 1048. Pt slowly responded to treatment and after 15 minutes from first dose of Benadryl patient had a decrease in redness and tongue swelling. Itching was slowly decreasing. At 1105 patient stated itching had subsided and by 1110 patient had a full return to baseline. Pt discharged at 11:15 home and pt communicated understanding that she was ok to drive and that she should follow up with PCP prior to receiving second dose of vaccine. Also, patient was able to communicate understanding of a rescue plan at home if symptoms should flare up again. Patient stated she did have an epi pen at home as well and note if she needed to she would use epi-pen and seek immediate medical attention after use.

Symptoms: By 1:30 am on 2/4/2021; patient was shaking uncontrollably. This continued about 1 1/2 hrs. then he could not get out of bed to go to the bathroom; thus wetting himself multiple times during the night; Patient was restless for about 5 hrs. but still could not get up even with me (his spouse) trying to help. He also could not understand how to help move his body to keep from falling out of bed (never fell out but I could not get his entire body on the bed until our daughter arrived around 9:30 am. It took our grandson who is 6'5" tall & 275 lbs. to help get patient up and walked him into our sunroon. throughout the day, patient could not stand without help & continued to urinate on himself. He could, however, feed himself breakfast & lunch. At 7:30 pm grandson and daughter had to walk patient into bedroom. patient could not walk without us telling him how and pushing his feet forward. It took both grandson and daughter to pull him onto the bed as he collapsed when he got to the bed. Patient slept until approximately midnight. at that time our daughter helped him to the bathroom and helped to change his underwear and tshirt which was wet from having a fever. Afterward, patient was able to go to the bathroom by himself but was very tired. He was able to walk outside with the help of a cane on 2/5/21 2 pm. On 2/6/21, patient complained of his back hurting and itching. We discovered he had shingles! Not a full body rash but shingles on his left side around his waist.

I had redness in the area (about 1-1.5 inch diameter) too that I have a picture of to show the injection location. Allergies: Egg allergy listed on vaccine form. Patient stated as a child he needed Epipen for this allergy. However, the past few years he has occasionally been eating eggs without any issues.

Symptoms: We were having a drive up COVID-19 vaccine clinic. Due to egg allergy he was instructed to wait for 30 minutes. About 15 minutes into his monitoring period medical staff was radioed for assistance. When I arrived at his vehicle he was alert, oriented, and breathing normally. He complained of sudden onset of bilateral anterior cervical lymphadenopathy. His face and neck had erythema. He stated due to eczema he normally had some redness but this was worse. At 12:21 he was given Epipen to left thigh IM. Vital signs HR 84, RR 18 BP 120/80 SP02 99%. At 12:25 he was given diphenhydramine 25mg IM left delt. Vital signs HR 92, RR 16, BP 160/100 SP02 99%. As Epipen was given, another staff was calling 911. Emergency services arrived at 12:33 and took over care. Patient was transported by ambulance to the University location. When he left via ambulance he was in stable condition.

Symptoms: Left arm started itching 1/5/21 and by 1/6 a red lump reappeared over the injection site that was firm and hot. I called the clinic to ask if that was normal as the initial shot symptoms had disappeared days earlier. They advised me to check with my doctor who asked me to come in to be checked. They felt like it was a localized reaction and to take Benadryl and Aleve for a few days. The lump and other symptoms gradually went away over several more days. They advised me to get the 2nd injection in the other arm.

Symptoms: Moderna COVID-19 Vaccine EUA 8-9 hours after my vaccine I fainted. I got up in the middle of the night, used the bathroom and fainted. I was not having stomach issue nor did I have a fever. I had eaten and drank enough fluids before and after the vaccine. I could feel that I was going to faint and started to sit down, luckily I did not fit my head only had a bruise on my forearm and hip. I woke up quickly and family members were nearby to help me slowly sit up. I did not faint anymore after this. I was very fatigued for the next 2 days. I contacted my health care provider, but did not need to be seen or treated. Medical History with fainting: I have fainted before, once in my teens when I had a very high fever and once after my first pap smear (vagal response).
