Symptoms: 10 days of intermittent moderate to severe headaches; nausea; muscle aches; joint pain; chills; moderate to severe fatigue which the fatigue was intermittent; unrelieved with anti-nausea medication or Tylenol and ibrofen. I missed 4 days of work. After about 14 -21 I felt recovered except for mild fatigue. Unable to return to baseline activity of running 5 days a week and surfing on weekends.

Symptoms: 6 hours following administration of the 1st dose in the Moderna COVID-19, patient experienced tongue swelling and felt small bumps on the top of her tongue. There was no pain with the event. The patient has experienced this event 1 time prior, at the time of onset/diagnosis of Hashimoto's disease; this was diagnosed in July 2017. Her thyroid levels were last checked in December 2020, . 1 hour after she noticed her tongue swelling, patient took 50 mg diphenhydramine PO and went to bed. When she woke up the next morning, the swelling had resolved.

Benadryl 25 mg PO was administered and patient was observed for another 15 minutes. No other adverse events observed or reported. Patient was ambulatory and able to drive herself home.

Symptoms: Beginning about 34 hours after the second vaccine dose, she noted itching of her posterior neck, upper back and across the shoulders, behind both ears, and the injection site in the right deltoid region. Initially she only noted itching in all of the above areas and slight redness of the injection site. She took a single 25 mg dose of diphenhydramine that night and went to bed. The next morning when she woke, her ears were swollen and red, she had blotchy red areas on her face, and increased redness and swelling of the injection site in the right deltoid. She also noted small red "bumps" on her lower legs and thighs bilaterally and across her abdomen as well. She also noted pruritis of her left arm at this time. She took a 10 mg dose of zyrtec this morning around 7:30 AM and then went to an urgent care for evaluation. At the urgent, she was given an IM steroid injection and a prescription for a 5-day course of prednisone 40 mg qday. She has noted significant improvement since receiving the steroid injection earlier today and her symptoms and the skin lesions are nearly completely resolved at this time.

EKG 12 LEAD UNIT PERFORMED/INTERPRETED (ED ONLY) Final Result (02/04 1932) EKG seen at 1920 interpreted the emergency department. Axis 57. Sinus rhythm ventricular rate of 82. No acute ischemic ST elevation noted. No acute T waves. QTc 405. Symptoms: ED COURSE / MEDICAL DECISION MAKING / TREATMENT PLAN: ED Course and Medical Decision Making: Patient evaluated. Peripheral IV established labs obtained. I do have concern for adverse reaction secondary to the vaccine as her symptoms started she said shortly before an hour after receiving the vaccine is started with tingling in her tongue throat and her body. She has minimal cardiac risk factors. Given she was tachycardic she cannot be perked out. EKG obtained showed no acute ischemic ST elevation or depression. Initial troponin was not elevated. Patient received 500 mL bolus of normal saline she deceived Benadryl and part of Pepcid. While the Benadryl infusion was going on patient reported feeling lightheaded and her heart rate was noted to increase to the 130s briefly. The medications were stopped, the Benadryl had already finished however the Pepcid was not completed. Her heart rate proved quickly and the lightheadedness resolved. Repeat EKG obtained did show some mild ST depression inferiorly. A repeat troponin obtained at 2 hours remain negative D-dimer was elevated to 91 therefore CTA chest was obtained. This showed no evidence of pulmonary embolism there is small groundglass opacity left upper lobe which is likely related to atelectasis do not feel that this is infectious in etiology patient has not had cough fever or chills. She remained on cardiac monitor here without any further tachycardia and her symptoms had resolved. I do believe patient's presentation is secondary to adverse reaction to the vaccination. She was advised to take Benadryl at home and may also try over-the-counter Pepcid at home for her symptoms. Did not feel that steroids were warranted at this time. She is to follow-up closely with her primary care provider. I did advise if she has new or worsening symptoms she should return to the emergency department and she expressed understanding and agreement with this plan.
