All occurred on 21Jan2021 18:00, the patient developed symptoms, such as aches all over his body, aches all over with muscles pains with muscle pains, headache, joint pain, chills, injection site pain on left arm, body slightly warm, felt slightly warm, feeling unwell. He had chills all night and could not get warm. The reporter had not taken the patient's temperature yet. The patient takes regular medications. The patient was on warfarin and standard medications. The reporter stated the vaccine cards were mixed up because they did not write their names on them and the vaccine lot numbers looks like they were mixed up. Someone probably made a mistake because one says EL3247 and the other says EL3427, so she is not sure which it is. The handwriting is different on the cards. The patient medical history was not reported. There were no concomitant medications. The patient experienced tested positive for COVID on 21Jan2021 after taking 1st dose of COVID vaccine. The patient tested positive for COVID after taking 1st dose of COVID vaccine and was calling to see what she should do about her 2nd dose. The reporter saw in the news that patients can wait up to 6 weeks after receiving the first dose of the vaccine to receive the second, she wants to confirm this information. The reporter was calling on behalf of herself as the patient. The patient got the Pfizer COVID-19 Vaccine last Friday (15Jan2021), and she took a COVID 19 PCR test on 21Jan2021, and the results came back as positive for COVID 19 today 22Jan2021. The patient was wondering what the recommendations are for getting the second dose if a patient tested positive for COVID 19, following having received the first dose. The patient was not complaining about the product, because the product has not had time to work yet. The patient believed the seriousness, it was medically significant because she was losing work over it. Causality: The reporter (patient) did not believe that she got COVID 19 because of the vaccine not working, she just did not think the vaccine had enough time to work before she was exposed. She had no other vaccines on the same day. After she got her first dose, there were a few side effects that she had but nothing was bad, so she was mad when she realized she had gotten COVID, therefore it was not the vaccines fault she got COVID. The reporter had some questions about how this vaccine works in the body. Her doctor thought that her now having tested positive for COVID, that might increase the immune response. The outcome of events was unknown. Relatedness between reaction COVID-19 PCR test positive and bnt162b2, was unrelated by source of assessment: primary source reporter per method of assessment: Global Introspection.; Sender's Comments: Based on the information currently available, COVID-19 and lack of efficacy of the suspected vaccine BNT162B2 cannot be completely excluded. However, individuals may not be protected until at least 7 days after their second dose of the vaccine. It is unlikely that the patient would have fully developed immunity due to the very short time lag between the first vaccine dose and the event onset.

Medical history included high blood pressure. The patient had no other patient history. The reporter took her mother, who's 83, and herself, on Saturday morning and got the COVID vaccine. Her mother woke up Sunday with a 99.8 temperature and she has been coughing real bad (cough) on 17Jan2021. The reporter asked if this was a side effect of the vaccine. This was her mother's first dose. Her underlying question was, after all this, she heard the second dose was worse, so the reporter wanted to know, should her mother get the second one if her mother has these symptoms, the reporter heard the second dose was worse and would give some symptoms. The only thing she was experiencing is her arm aching. But it was just like when she got a flu shot, her arm hurts happened. Husband went to the hospital on 16Jan2021 and they discharged him and he went back on 17Jan2021 and did a magnetic resonance imaging (MRI) and diagnosed with sciatica and gave him oxycodone 10mg 1 mg 1 four times a day as needed, cyclobenzaprine 10 mg 1 every 8 hours, and gabapentin 100 mg 2 three times a day and a prednisone 6 pack. Reported not getting any better so took him to pain management and could not get comfortable and screamed the whole way over which was 20 minute ride and went in the office in a wheelchair. They gave him a steroid and told him to wait 1/2 hour. When leaving he experienced the worst spasm ever and they gave him Toradol and then additional steroid injections. Husband was able to walk out with a walker. Can only lay down, can not sit up and is bedridden. Spasms have stopped at the time of the phone call but still in pain. Medical history included Lupus may make her immunocompromised which diagnosed in 2011, Raynaud's, hypertension which was completely managed, irritable bowel and reflux. All were diagnosed prior to the vaccine. Family medical history was none. There were no additional vaccines administered on same date of the Pfizer suspect. Concomitant medication included hydroxychloroquine for Lupus for many years, tadalafil (CIALIS) took for years for secondary reynauds associated with lupus, amlodipine from Jan2019 for blood pressure abnormal. The patient did not receive any other vaccines within 4 weeks prior to the COVID vaccine. The patient only had the first vaccine and at the time she received it, she got hot and flushed and went bright red in the face. That lasted 2 hours. She was literally bright red on her face. She got it Friday at 2:27 pm on 15Jan2021. Friday night, she slept 14 hours, which was unusual. She only got out of bed to walk her dogs. Saturday (16Jan2021), she felt funny and she started to get hot and chills at the same time. She took her temperature and temperature started in mid 99 degrees Fahrenheit and popped up to 100 degrees Fahrenheit. Then same thing again Saturday night. She slept 14 hours, which was unusual. Sunday (17Jan2021), she felt crummy. Her temperature spikes to 100.3 on Sunday. She contacted her manager who advised her to contact Employee Health the next morning and employee health told her that if her temperature spiked again on Monday to get a COVID test. She said it never went above 99 degrees Fahrenheit. Today (21Jan2021) was the first day she did not feel crummy. The sleepiness improved but only because she had to go to work. She was still tired. Feeling funny lasted 2-3 days. Her temperature on Saturday (16Jan2021) was 100 degrees Fahrenheit and on Sunday (17Jan2021) was 100.3 degrees Fahrenheit. She never took her temperature on 15Jan2021. The temperature had improved, but it was still hanging out around 99.4 and 99.6. Feeling crummy persisted through yesterday (20Jan2021). Today (21Jan2021) was the first day she felt normal again. She felt crappy all week. She took a fair amount of medication and did not know if it was relevant or not. She had other medications and had been on them for a long time. No ER or physician's office required. She contacted with employee health but had not seen a provider. She planned to discuss with her provider because she wanted recommendation if she should receive vaccine 2. She realized it was a judgment call because she was only 50% covered after having the first dose. She was not sure if she will have a worse reaction with the second. Most people she talked to have no reaction but a significant reaction to the second one. She did not have any information on any of those that had a significant reaction. A 41-year-old female received her second single dose of BNT162B2 (Pfizer-BioNTech Covid-19 vaccine, lot EL3302) at left arm 08:30AM on 20Jan2021 for Covid-19 immunization. The patient had first shot 08:00AM on 30Dec2020 at right arm and experienced sore arm within 12hrs, fatigue, joint pain, skin itch, and enlarged right supraclavicular lymph node. The patient had no known allergies and other medical history was not reported. Concomitant medications included metformin. The patient experienced sore arm within 4hrs, fatigue, joint pain, fever around 12:15PM on 20Jan2021. No treatment received for the events. The patient had no Covid prior vaccination and not Covid tested post vaccination. The event did not resolve at the time of reporting.
