Lab Data: 9/17/2021 COVID19- positive 9/17/21 chest xray: Bibasilar tissue prominence may reflect interstitial inflammation/infection or interstitial edema. 9/18/21 CT chest: Findings consistent with emphysema with mild traction atelectasis moving the mediastinum to the right. In addition there is severe fibrotic changes of the right lung with a superimposed pneumonia. 10/13/21 chest xray: COPD changes. Extensive consolidation within the right lung redemonstrated. Overall, this is probably mildly worse when compared to the prior study specially within the right lower lung. Left basilar airspace disease could represent atelectasis or additional pneumonia. This also appears mildly worse.

Symptoms: This is an elderly 72-year-old female who is an ongoing smoker with a known history of COPD on home O2 at the nursing home, possibly 2 L p.r.n. was out smoking and then had an unwitnessed syncopal episode. Was sent to the emergency room on 9/17/21 where she was found to be hypotensive and in respiratory distress. Her hemoglobin was found to be only 4.3. Patient was started on a BiPAP and then the hospitalist service was called for admission. Patient denies any history of epistaxis, hemoptysis, hematemesis, melena, hematochezia, hematuria, and she is not on any blood thinning medications. Her rectal exam revealed OB negative stool. The patient is denying abdominal pain. She does complain of feeling lightheaded. She does have a history of chronic anemia, but her baseline hemoglobin runs around 9.3, which was there in February of this year. At that point of time, her MCV was 88.6. Currently, her H and H is 4.7 and 18.5 with an MCV of 61.5. I have ordered stat anemia workup on her and a CT abdomen and pelvis and chest as well. Her COVID test did come back as positive. She will be going up to the IMC as an inpatient. Started her on Rocephin because her urinalysis is abnormal and the chest x-ray, there is a possibility of some COVID related pneumonia as well. Patient rececived and completed all 5 doses of remdesivir and 10 days of dexamethasone. Placed on ampicillin on 10/2/21. Discharged on 10/14/2021

Symptoms: Patient began to feel dizzy 1 minute or less after given first dose of Moderna. She then became unresponsive. Called 911 . Her mother held her upright in chair until it was safe to move her to floor. And I cradled her to monitor that she still had a pulse and was breathing. Continued to talk with her while waiting for ambulance to arrive. She did not respond to verbal interaction, she did have a pulse and was breathing, however it was shallow. Her eyes were open but unresponsive. We moved her to the floor as soon as it was safe after about 10 minutes. Once we moved her to the floor she became responsive once again. gave her a bottle of water and at that time the EMT arrived. They checked her out and she was able to answer their questions. They completed their care for her and left her to her mother's care.

Symptoms: I was diagnosed with severe ulcerative colitis about two months after my second vaccine. I started having symptoms about 1 month post vaccination. I ultimately was hospitalized for ten days with severe bleeding from ulcers in my colon. I am now on Remicade for my ulcerative colitis. Admission Date: 10/6/2021 Discharge Date: Oct 9, 2021 PRESENTING PROBLEM: Hyponatremia Hypophosphatasia Encephalopathy COVID-19 Acute respiratory failure due to COVID-19 HOSPITAL COURSE: This is a 66-year-old gentleman with a history of progressing right chest mesothelioma, as well as type 2 diabetes mellitus. He has had some progressive memory problems and word-finding problems over the past months, but developed acute worsening mental status in the 3 days prior to admission. When he came to the emergency room he was found to have fever and was borderline hypoxemic. He was found to have COVID-19 infection. He had anemia with a hemoglobin of 10.4, and subsequent anemia workup revealed an iron saturation of 3% consistent with iron deficiency anemia, as well as of vitamin B12 level of 189 consistent with vitamin B12 deficiency. He was treated with IV iron sucrose, of which he received 2 doses of 200 mg during his hospital stay. He was also treated with 1000 mcg of vitamin B12 IM. For his COVID-19 infection he was started on remdesivir and dexamethasone. He developed acute agitated confusion after admission requiring IV Haldol. This was thought to be a steroid psychosis, and it resolved with discontinuation of the IV Decadron. He is on oral Decadron 2 mg daily as an outpatient which was resumed. He also seemed to develop some confusion from remdesivir dosing so this was stopped as well. By discharge she was at his baseline mental status. He was breathing comfortably without oxygen. For his altered mental status he had a head CT on admission which was negative, and also an MRI of the brain with and without contrast which was normal except for evidence of small-vessel cerebrovascular disease. Discharge medications include the addition of ferrous sulfate 325 mg daily and vitamin B12 500 mcg daily. Follow-up recommendations include consideration Neurology consultation to evaluate cognitive dysfunction. For his vitamin B12 deficiency and iron deficiency recommend follow-up complete blood count, B12 levels, and iron levels to determine if IM B12 maintenance is necessary or IV iron infusions are necessary.

Current Illness: 9.22.21: Office visit with Pulmonary medicine: 66-year-old male history of prior tobacco use, pleural mesothelioma with right pleural effusion and Pleurx catheter on immunotherapy Who presents for follow-up. Symptoms: 1) Approximately 6 hours after receiving the injection, I had burning sensations in the inner thigh of my left leg, and between the arm pit and lung area of my left side. Fever HPI & Medical Decision Making HPI: Patient is a 71 y.o. male presenting to the ED with fever and mild confusion. Patient is really unable to report why he is here, is alert to himself and place. Recently had a right foot biopsy on October 1 out of concern for osteomyelitis, biopsy negative, continued on amoxicillin clavulanate for 3 weeks as an outpatient in consultation with infectious disease and podiatry. Conversation with wife over the phone notes that patient had been feeling fine night before presentation, awoke with a fever and confusion per daughter, who recommended EMS be called. MDM: This is a 71-year-old male presenting with fever and confusion per family. Hemodynamically stable on arrival on exam, transient dip to the mid 90s systolic that has stabilized with liter of crystalloid. Undifferentiated 71-year-old with fever will work broadly up for sepsis, the wound on his right heel appears to be healing adequately with no signs of infection. Confusion may also be seen by electrolyte abnormalities, which will be assessed for on his sepsis work-up. Work-up broadly negative for changes from baseline, with a stable hemoglobin of 10.9, stable creatinine of 1.27, and a bland urinalysis with a normal lactate. He is COVID-19 positive, which likely explains his temperature. ED to Hosp-Admission to Current 10/11/2021 - present (3 days) COVID-19 Principal problem Assessment and plan: 71-year-old male with pneumonia secondary to COVID-19 despite vaccination x2, patient with multiple medical problems including untreated OSA, history of A. fib on chronic warfarin, subtherapeutic INR, severe disability with ambulatory dysfunction, DM2 w hyperglycemia on chronic insulin-- Plan is admission for O2, remdesivir and decadron Insulin long and short acting and ISS lovenox and coumadin since subtherapeutic (and high risk for PE) (once therapeutic on coumadin,stop lovenox) dispo--PT/OT and likely SNF (severe tremor and slow to move)-almost parkinsonian features Cont augmentin for infection on R heel DNR (discussed w pt and daughter) Chief complaint: Cough and shortness of breath HPI: This is a 71-year-old male who had 2 vaccines for Covid months ago was hospitalized recently for right foot infection (osteomyelitis neg per biopsy)- discharged on a 3 wk course of augmentin (end date 10/25) -came back to the hospital 4 days after his discharge with cough and fever tested positive for Covid was discharged home because of normal oxygen saturation comes back in today because he has continued cough chills shortness of breath and his oxygen saturation was 84% on room air currently stable on 5 L of oxygen.

Symptoms: Hospitalized 10/9/2021; COVID-19 positive 10/9/2021; fully vaccinated Patient with past medical history significant for hypertension, hyperlipidemia, type 2 diabetes on insulin, OSA, BPH, chronic pain, history of type B aortic dissection with mild aortic aneurysm who presented for this hospitalization with fatigue, fever, cough, shortness of breath for the past 2 days. Symptoms began on 10/07/2021. Patient is fully vaccinated with Moderna vaccine, last dose given March 2021. Patient attended his son's wedding on 10/09/2021, and presented with the above symptoms thereafter. Patient endorsed decreased oral intake over the past couple of days with these symptoms, no changes with urination, no diarrhea, no abdominal pain, nausea, vomiting or constipation. No chest pain, subjective fevers without checking temperatures at home. Patient denied all other review of system questioning. COVID-19 PCR positive in the emergency department. CT angiogram negative for PE, showed stable aortic dissection and aneurysm, positive for airspace disease consistent with COVID-19 pneumonia, as well as some subcarinal and mediastinal reactive lymphadenopathy consistent with infection. Patient desaturated into the mid 80% range on room air, required 4 L nasal cannula for stable pulse ox measurements. Chief Complaint Patient presents with ? WEAKNESS ASSESSMENT/PLAN: Acute hypoxic respiratory failure due to: COVID-19 pneumonia: In the context of Complete vaccination against COVID-19 with Moderna: Sepsis due to above - onset of symptoms 10/07/2021, will be out of isolation on 10/16/2021 if needing institutional protocol - heart rate 103, respiratory rate 31, source is pneumonia from COVID-19 - CT angiogram negative for PE, noting chronic type B aortic dissection and mild aneurysm, positive for classic airspace disease consistent with COVID-19 pneumonia - requiring 4 L nasal cannula, wean as tolerated - prone as tolerated - check baseline D-dimer and monitor for 3 days - CRP daily for 3 days to monitor for worsening inflammation - lovenox daily for VTE ppx - check lactic acid per sepsis protocol - discussed benefits and risks of remdesivir, patient declined - received decadron in the ED, continue daily for total 10 days - cough, deep breathe, incentive spirometry. Note from 10/11/21: Chief compliant: COVID 19 infection. ASSESSMENT/PLAN: COvid 19 infection: Acute hypoxic respiratory failure due to above: Sepsis due to above: Oxygen requirements are increased and now he is on high-flow nasal cannula. CRP level increased. Given these 2 findings discussed about remdesivir patient agreed to receive. Start remdesivir. Continue dexamethasone day # 3.. Encourage prone position and Spirometer. 10/14/21: ASSESSMENT / PLAN: No new Assessment & Plan notes have been filed under this hospital service since the last note was generated. Service: General Medicine COVID 19 pneumonia Acute hypoxic respiratory failure Sepsis - Transferred to ICU as CC2 on 10/12. Now on HFNC FiO2 100%, still requiring NRB - CRP increased, now trending down - Remdesivir x5 days. Decadron 20mg x5 then 10mg x2 per ICU recs - Cont CAP coverage for now - Encourage prone position and Spirometer - CXR 10/12 showing worsening severe COVID19 -pneumonia. Repeat CXR today.
